摘要
目的:研究盐酸关附甲素注射液对人体的血流动力学作用。方法:经颈静脉或股静脉送入Swan-Ganz导管至肺动脉主分支内,进行血流动力学监测。盐酸关附甲素注射液负荷量4mg·kg-1,5min内静脉匀速注射,继以维持量40μg·kg-1·min-1静脉泵入90min。观察10例健康受试者用药前、用药中和停药后12h内的血流动力学参数的变化。结果:用药前后血压、肺动脉压、肺动脉楔压(PAWP)、右房压、肺循环阻力无明显变化。心输出量及心脏指数在维持量90min时轻度降低,每搏输出量及每搏指数同时轻度降低,体循环阻力升高。但以上参数的均值在正常范围之内。结论:虽然盐酸关附甲素注射液可使部分血流动力学参数发生轻度改变,但受试者对盐酸关附甲素在血流动力学方面耐受较好。
OBJECTIVE: To evaluate the hemodynamic effects of Guan-Fu base A (GFA), a new antiarrhythmic drug, in human. METHOD: Ten normal volunteers underwent the invasive hemodynamic study. A Swan-Ganz thermodilution catheter was introduced into main branch of pulmonary artery percutaneously. After the basic hemodynamic measurement, a bolus of 4mg.kg-1 of GFA was given intravenously in 5 minutes followed by maintenance infusion of 40ug.kg-1.min-1 for 90 minutes. Hemodynamic measurement was repeated immediately after the bolus and through the period of i.v. infusion till 12 hours after the termination of administration. RESULT: After the administration of GFA, blood pressure, pulmonary pressure, right atrial pressure and pulmonary arterial wedge pressure were not affected. Cardiac output, cardiac index, stroke volume and stroke volume index decreased slightly at 90 minutes after the maintenance dose (P<0.05). Systemic vascular resistance was increased by GFA after 90 minutes infusion. The mean values of above parameters were still within the normal range. CONCLUSION: Although some hemodynamic parameters changed slightly by GFA after 90 minutes infusion, the medicine was welltolerated in patients with normal cardiac function.
