摘要
目的评估双环醇治疗抗风湿药物导致肝损害的疗效和安全性。方法收录2012年1月-2012年6月北京友谊医院住院的风湿病患者132例,均被诊断为药物性肝损伤,随机分为两组:治疗组65例,服用双环醇片,25 mg,3次/d;对照组67例,服用葡醛内酯片,150 mg,3次/d,疗程均为12周。分别于治疗前及治疗后4周、12周检测两组患者血清ALT、AST、ALP和TBIL水平,并记录血常规、尿常规、肾功能等指标。结果治疗12周后各项肝功能指标明显下降,与基线期比较差异均有统计学意义(P均<0.01),治疗组与对照组肝功能各项指标值分别为:ALT(33.34±9.19)U/L,(57.94±24.16)U/L(P<0.01);AST(36.11±13.48)U/L,(64.78±48.99)U/L(P<0.05);ALP(56.08±20.86)U/L,(52.96±19.43)U/L(P>0.05);TBIL(12.52±7.68)μmol/L,(13.78±8.81)μmol/L(P>0.05);治疗组总有效率明显优于对照组,差异有统计学意义(86.15%,65.67%,P<0.01)。两组患者均未出现严重不良事件。结论双环醇能够有效治疗抗风湿药物导致的肝损伤,且安全性好。
Objective To evaluate the efficacy and safety of bicyclol in treatment of patients with liver injury caused by anti-rheumatic agents.Methods The enrolled 132 patients from Beijing Friendship Hospital with rheumatic diseases and drug-induced liver injury were randomly divided into two groups：therapy group （bicyclol 25 mg tid,n =65） and control group （glucurolactone 150 mg tid,n =67）.The indexes including liver function,blood routine,kidney function,urine routine were tested and compared before and after 4 weeks,12 weeks treatment.Results After 12 weeks treatment,the levels of liver function were decreased obviously （P ＜ 0.01）,and the indexes of both groups were respectively indicated below：ALT（33.34 ±9.19） U/L,（57.94 ±24.16） U/L（P ＜0.01） ;AST（36.11 ± 13.48） U/L,（64.78 ± 48.99） U/L （P ＜ 0.05） ; ALP （56.08 ± 20.86） U/L,（52.96 ± 19.43） U/L （P ＞ 0.05） ; TBIL （12.52 ± 7.68） μmol/L,（13.78 ± 8.81）μmol/L（P ＞ 0.05）.The overall response rate of therapy group was higher than that in control group（86.15％,65.67％,P ＜ 0.01）.No obvious adverse reactions happened during treatment.Conclusion It is available and safe for treating drug-induced liver injury caused by anti-rheumatic agents with bicyclol.
