摘要 目的:采用Meta分析的方法探讨超声造影对肾囊性病变的诊断价值。资料与方法:以contrast enhanced,US,cystic,renal mass,超声造影,囊性肾癌,肾囊性病变等为关键词,检索cochrane图书馆、PubMed、Embase、中国知网、万方数据服务平台等数据库中的中英文文献。使用Meta-Disc软件,对采用的文献数据进行异质性检验,计算其汇总灵敏度、特异度、阳性似然比和阴性似然比,描绘受检者工作特征曲线(SROC),通过曲线下面积评估超声造影诊断肾囊性肿块的价值。使用QUADAS条目进行文献质量评价。结果:经检索总共采纳7篇文献,其中4篇为中文,3篇为英文。按照随机效应模型合并,结果其汇总灵敏度、特异度、阳性似然比、阴性似然比和诊断试验比值比分别为0.92、0.84、4.54、0.09、69.9,SROC曲线下面积为95.2%。结论:超声造影对肾囊性病变具有较好的诊断和临床应用价值。 Objective: To assess the diagnostic value of contrast enhanced ultrasound technology in the diagnosis of cystic renal masses by Meta-analysis. Methods: Contrast enhanced ultrasound and cystic renal masses were selected as the key- words. Searching the Cochrane library, PubMed, Embase, CNKI and the WANFANG DATA, the original data were abstracted and performed heterogeneity test with the Meta-Disc software. The weighted sensitivity, specificity, positive likelihood ratio and negative hkehhood ratio were calculated, as well as the summary receiver operating characteristic (SROC) curve. Further esti- mated the diagnostic value of contrast enhanced ultrasound in the research of cystic renal masses by calculating the area un- der the curve. The quality of evidence in researches were evaluated by QUADAS items. Result: A total of 7 studies including four Chinese papers and three English articles were adopted for Meta-analysis. According to the random effects model, the weighted sensitivity, specificity, positive likelihood ratio, negative likelihood ratio,diagnostic odds ratio were 0.92, 0.84, 4.54, 0.09, 69.9, respectively; and the area under the curve was 0.952. Conclusion: Contrast enhanced ultrasound technology has a good diagnostic and clinical value in the study of cystic renal masses.
