发散式冲击波循经取穴治疗腰椎间盘突出症疗效观察 被引量:20

EFFECTS OF RADIAL SHOCK WAVE THERAPY ON ACUPOINTS ALONG THE CHANNEL IN THE TREATMENT OF LUMBAR DISC HERNIATION

摘要 目的:评价发散式冲击波(radial shock wave therapy,RSWT)循经取穴治疗腰椎间盘突出症的疗效。方法:回顾性分析发散式冲击波循经取穴治疗的腰椎间盘突出症患者39例(治疗组),与传统保守疗法治疗腰椎间盘突出症患者32例(对照组)进行比较。分析两组间疼痛视觉模拟评分(visual analogue scale,VAS)、日本骨科学会(Japanese Orthopaedic Association,JOA)下腰痛评分、住院天数及疗效。结果:冲击波首次及第3次治疗后,治疗组VAS值显著降低(P<0.01),并低于对照组(P<0.01)。两组治疗后2周及3个月随访,下腰痛JOA评分均较治疗前显著升高(P<0.01),两组间比较差异无统计学意义。治疗组治疗时间明显低于对照组(P<0.01)。治疗结束后,治疗组治愈率明显高于对照组(P<0.05)。结论:发散式冲击波循经取穴治疗腰椎间盘突出症在降低VAS评分和治疗时间、提高治愈率方面优于传统治疗方法。该疗法值得临床推广和应用。 Objective: To evaluate the curative effects of radial shock wave therapy (RSWT) on acupoints along the channel in the treatment of lumbar disc herniation (LDH). Methods: This retrospective study analyzed 39 LDH patients treated with RSWT (treatment group) and 32 cases treated with conservative therapies (control group). VAS scores, JOA scores, the therapeutic time and cure rate were recorded. Results: Compared with before treatment and the control group, VAS was lower in the treatment group after the first and the third treatment session (P 〈 0.01). The JOA scores of both groups increased at 2 weeks after treatment and 3 months followup (P 〈 0.01), and there was no significant difference between the two groups. The therapeutic time in the treatment group was lower than that of the control group (P 〈 0.01). The cure rate in the treatment group was relatively higher than that of the control group (P 〈 0.05). Conclusion: RSWT on acupoints along the channel has advantages over the conservative therapies in relieving pain, reducing therapeutic time and increasing cure rate. RSWT is worth to clinical application and popularization.
