低负压吸痰应用于脑卒中昏迷建立人工气道患者的效果 被引量:7

Application Effects of Low Negative Pressure Suction Applied to Stroke Coma Patients with Artificial Airway

摘要 目的探讨不同负压吸痰对脑卒中意识障碍行人工气道管理患者的影响。方法 2011年2月至2012年2月,便利抽样法选取在海军总医院治疗的80例脑卒中行人工气道患者为研究对象,按其入院时间的先后将其分为对照组(n=40)和观察组(n=40)。对照组患者接受常规吸痰,吸痰压力为0.05MPa;观察组为低负压吸痰组,吸痰压力为0.025MPa。记录两组患者吸痰前后收缩压、舒张压、心率、血氧饱和度的变化情况及两组患者在实验的第3、7、10、14天血白细胞数、呼吸道压力的变化。结果低压力吸痰对患者吸痰前后的收缩压、舒张压、心率、血氧饱和度影响较小,差异均有统计学意义(均P<0.05);两组患者血白细胞数、呼吸道压力的差异无统计学意义(P>0.05)。结论降低吸痰压力对于脑卒中患者仍可实现有效的吸痰,维持患者血压平稳,适用于脑卒中患者的气道管理。 Objective To explore the application effects of different negative pressure suctions for stroke coma patients with artificial airway. Methods From February 2011 to February 2012, a total of 80 cases of stroke patients with artificial airway were selected and divided into observational group (40 cases) and con- trol group (40 cases) according to the admission time. Patients in the observational group were managed by low negative pressure aspiration with sputum suction pressure at 0. 025 Mpa, while patients in the con- trol group received routine sputum with sputum suction pressure 0.05 MPa. The changes of systolic pres- sure, diastolic pressure, heart rate and oxygen saturation between before and after the sputum suction, and the white blood cell count and airway pressure respectively on 3rd, 7th, 10th and 14th days in two groups were recorded and compared. Results Low pressure aspiration of sputum suction had significantly less influence on the systolic and diastolic pressure, heart rate and oxygen saturation. No significant difference was found in white blood cell count and airway pressure between two groups. Conclusion Decreasing sputum suction pressure for stroke patients could equally achieve effective suctioning,with the additional advantage to maintain their blood pres- sure stable. It was suitable for airway management of patients with stroke.
