摘要
目的探讨双吻合器技术在超低位直肠癌保肛术中的应用价值及注意事项。方法回顾性分析2008年6月—2012年5月收治的30例超低位直肠癌患者,按TME原则,用双吻合器技术行超低位直肠癌切除、结直肠或结肠肛管吻合保肛手术。结果全组成功保肛26例,预防性横结肠造瘘3例。术中发现直肠阴道瘘者1例,术后第六天、第十天各发现1例直肠阴道瘘;吻合口瘘2例,尿潴留2例;淋巴瘘1例;无手术死亡、性功能障碍、吻合口出血及狭窄病例。结论对于超低位直肠癌,只要严格掌握手术适应证,遵循TME原则、强调无瘤原则,采用双吻合器技术实行保肛手术是安全可靠的,保肛成功率高,显著提高了患者的生活质量。
Objective To evaluate the value and application attentions of double stapling technique（DST） in the treatment of ultra- low rectal anastomosis with the anus- preserving procedure. Methods In accordance with the principle of total mesorectal excision, from June 2008 to May 2012, we treated 30 patients with ultra- low rectal cancer by using the DST with the anus- preserving procedure. Results Among 30 cases, twenty- six patients were treated successfully with a anus- preserving procedure, three patients were treated with preventive transverse colostomy. In which one patient developed rectovaginal fistula in the operation, two patients with rectovaginal fistula in the six and ten days after the operation, two patients with anastomotic leakage and two patients with dysuresia and one patient with lymphatic fistula. There were no complication of death, sexual function disorder, anastomotic bleeding and anastomotic stenosis. Conclusions DST in the treatment of ultra- low rhctal anastomosis with the anus - preserving procedure is safe and reliable with high rate of anus- preserving procedure rate in accordance with the principle of total mesorectal excision, mastering indication of operation seriously and emphasizing tumor- free principle. It could improve the life quality of the patients.
