摘要
目的:分析脐动脉血气分析与新生儿窒息的临床相关性,探讨脐动脉血气分析在新生儿窒息诊断中的应用价值。方法:选择2010年7月~2012年9月,在我院分娩的单胎足月新生儿150例,以上入选新生儿均进行脐动脉血气分析与出生后1min Apgar评分,分析新生儿脐动脉血气分析结果与出生后1min Apgar评分的相关性,并探讨其在新生儿窒息诊断中的应用价值。结果:新生儿脐动脉血PO2、pH值、BE值与新生儿Apgar评分,呈正相关性(P<0.05);脐动脉血PO2与新生儿Apgar评分,呈负相关性(P<0.05);重度窒息组与轻度窒息组和正常组的脐动脉血气分析结果,差异均具有(高度)统计学意义(P<0.05,P<0.01);重度窒息组分别与轻度窒息组和正常组的pH值<7.20与pH值>7.25患儿例数比较,差异均具有(高度)统计学意义(P<0.05,P<0.01)。结论:在新生儿窒息的临床诊断中,脐动脉血气分析能够弥补传统Apgar评分的不足,对新生儿窒息的早期诊断与预后判断具有重要的临床意义。
Objective :To analyze the clinical application of umbilical arterial blood gas analysis and neonatal asphyxia and explore the application value of umbilical arterial blood gas analysis in the diagnosis of neonatal asphyxia.Method:Collect 150 neonates ,they were performed the umbilical arterial blood gas analysis and lmin Apgar scores at birth,analyze the clinical application of mnbilical arterial blood gas analysis and neonatal asphyxia and explore the application value of umbilical arterial blood gas analysis in the diagnosis of neonatal asphyxia.Result:There was a positive correlation (P〈0.05) in Umbilical arterial PO2, pH value, BE values and neonatal Apgar score;there was a negatively correlated (P〈0.05)in umbilical arterial POz and neonatal Apgar scores;there was a (highly)significant (P〈0.05, P〈0.01 ) in the umbilical arterial blood gas analysis results between severe asphyxia group and the mild group and normal group;there was a(highly)significant(P〈0.05 ,P〈0.01 ) in the number of pH〈7.20 and pH〉7.25 case between severe asphyxia group and the mild group and normal group.Conclusion:In the diagnosis of neonatal asphyxia,the umbilical arterial blood gas analysis could make up for the deficiencies of Apgar scores and improve the diagnosis rates and cure rates of neonatal asphyxia.
