生物羊膜在鼓室成形术中的应用探讨 被引量:7

摘要 目的探讨生物羊膜作为鼓膜及创面修复材料在鼓室成形术中的应用价值。方法选择2010年1月至2012年12月间我科收治的32例中耳胆脂瘤患者,在显微镜下应用生物羊膜作鼓膜及创面修复材料,行乳突切开并鼓室成形术。10天后去除外耳道填塞物,观察鼓膜情况,随访6个月。结果除2例术后羊膜脱落坏死而穿孔未愈、行二次鼓膜修补术外,其余30例全部一期愈合、干耳,术后3个月行纯音测听,30例患者气导听阈均有提高,与术前纯音听阈值相比,各频率段差异有统计学意义(P<0.01)。结论生物羊膜可成为鼓室成形术较为理想的移植修复材料,生物羊膜材料可规模化、标准化生产,操作简单方便,成活率高,值得推广。 Objective To observe the clinical value of biological amnion as the tympanic membrane and wound repair material in tyrnpanoplasty. Methods 32 patients (32 ears) with cholesteatoma otitis media in our department from January 2010 to December 2012 were performed the open method tympanoplasty with mastoid obliteration and reconstructed by using human biological amrtion. The ear canal filling were removed 10 days after surgery, and the tympanic membrane situation was observed. All cases were followed up for 6 months. Results All cases were healed except 2 cases of perforation inducing by postoperative necrosis and denudation of biological amnion. Other 30 cases had primary healing, and achieved dry ear. The pure tone test results after 3 months showed that 30 cases had improved audibility threshold. Compared with the pure tone audiometry before surgery, the difference of each frequency was statistically significant (P 〈0.01). Conclusions Biological amnion can be regarded as the ideal transplantation and repair material. It is easy to obtain, operate and produce large-scale and regularly,, therefore, it is worthy of application.
