摘要
目的:系统评价荷丹片治疗原发混合型高脂血症的有效性和安全性。方法:计算机检索Cochrane图书馆临床对照试验资料库、PuhMed、Embase、维普期刊、万方电子期刊、中国知网(CNKI)、中国生物医学文献数据库(CBM),纳入治疗组服用荷丹片,对照组服用他汀类降脂药治疗原发混合型高脂血症的随机对照试验。采用RevMan 5.1软件对数据进行分析,并绘制森林图。结果:共纳入7个研究,550例原发混合型高脂血症患者纳入研究质量均为C级。在联合用药组中,荷丹片联合他汀组与他汀组比较,降低TC[MD=-0.33,95%CI(-0.51,-0.14)]、降低LDL[MD=-0.62,95%CI(-0.93,-0.31)]和升高HDL[MD=0.33,95%CI(0.29,0.37)]的效果差异均有统计学意义(P<0.01),降低TG[MD=-0.95,95%CI(-2.11,0.21)]的效果差异无统计学意义(P>0.05)。在单药组中,荷丹片组与他汀组比较,在降低TC[MD=-0.08,95%CI(-0.49,0.34)]、降低TG[MD=-0.67,95%CI(-1.81,0.47)]、降低LDL[MD=0.01,95%CI(-0.47,0.48)]、升高HDL[MD=0.17,95%CI(-0.02,0.35)]的效果差异均无统计学意义(P>0.05)。结论:荷丹片联合他汀类药物能显著降低原发混合型高脂血症患者的TC、LDL-C水平,荷丹片单独使用与他汀类药物相似。本系统评价纳入研究的质量和数量有限,上述结论尚需更多高质量、大样本、长期随访的随机对照试验加以验证。
To assess the effect and safety of Hedan tablets on lipid level in the patients with essential combined hyperlipidemia. Method：Cochrane Central Register of Controlled Trials,Pubmed,Embase,VIP,Wangfang Database,CNKI and CBM were searched.Randomized controlled trials（RCTs） were selected with Hedan tablets in the experimental group and statins alone in the control group.The data were evaluated by RevMan 5.1 software,and the forest plots were drawn.Result：Totally 7 articles involving 550 patients were included.All the articles were regarded as low quality according to Cochrane quality evaluation criteria.The result of met a - analysis showed the combination application of Hedan tablets and statins was more effective on TC[MD = - 0.33,95% CI（-0.51,-0.14）,P0.01],LDL-C[MD= -0.62,95%C/（-0.93,-0.31）,P0.01]and HDL-C[MD =0.33,95%CI （0.29,0.37）,P0.0]than statins alone,while there was no significant difference between the combination group and the control group in TG[MD - -0.95,95%C/（-2.11,0.21）,P0.05].No significant difference was shown between the experimental group and the control group in TC[MD = -0.08,95%C/（-0.49,0.34）,P 0.05]）,TG[MD = -0.67,95%CI（- 1.81,0.47）,P 0.05],LDL-C[MD =0.01,95%CI（-0.47,0.48）,P 0.05]and HDL-C[MD =0.17,95%CI（-0.02,0.35）,P0.05]. Conclusion：Currently available evidence shows that the effect of Hedan tablets is the same as that of statins,and the combination of Hedan tablets and statins is more effective on TC and LDL-C.However,clue to lack of high quality included studies,more studies with better quality,larger population and longer-term follow-up should be conducted.
