摘要
目的总结艾滋病合并马尔尼菲青霉菌病(PSM)的影像学特点。方法对62例艾滋病合并PSM患者的影像学资料进行回顾分析。结果艾滋病合并PSM患者的胸部影像学表现复杂多样。单发或多发结节及结节内有空洞者29例(46.8%),粟粒状结节14例(22.6%),斑片、絮片状模糊影15例(24.2%),其中有9例合并有结节或粟粒状结节,双肺磨玻璃样改变5例(8.1%);胸水12例(19.4%),其中有9例合并有肺部改变;肺门和(或)纵隔淋巴结肿大41例(66.1%),仅有6例表现为纵隔淋巴结肿大而肺部无异常,但均合并有肝和(或)脾肿大、腹腔淋巴结肿大、颜面部脐凹样皮疹;合并肝和(或)脾肿大27例(43.5%);合并腹腔淋巴结肿大(以低密度为主)13例(21.0%);少见表现有合并心包积液、气胸等。结论艾滋病合并PSM的胸部影像学改变主要表现为结节和(或)结节空洞、粟粒状结节、肺门和(或)纵隔淋巴结肿大、肝脾肿大及腹腔低密度为主的淋巴结肿大。
Objective To summarize the thoracic radiological features of AIDS complicated with Penicilliosis Marneffei （PSM）. Methods Radiological findings of 62 cases AIDS complicated with PSM were retrospectively analyzed. Results The thoracic radiological findings were complex and various. Twenty-nine cases （46.8%） showed solitary or multiple small nodules, and cavities could be seen in those nodules; 14 cases （22.6%） showed miliary nodules; 15 cases （24.2%） showed patchy loci or flaky opacities, and 9 of them were associated with nodules or miliary nodules; 5 cases （8.1%） showed frosted glass like change; 12 cases （19.4%） showed pteural effusion, and 9 of them also had abnormal pul- monary changes; 41 cases （66.1%） had hilar and （or） mediastinal lymphadenopathy. Among them, only 6 cases had mediastinal lymphadenopathy and hepatosplenomegaly, peritoneal lymphadenopathy or umbilical-like rash in the facial area, but had no pulmonary changes; 27 cases （43.5%） were associated with hepatosplenomegaly, hepatomegaly or splenomegaly; 13 cases （21.0%） were associated with peritoneal lymphadenopathy （most of them were low density）. Pericardial effusion or pneumothorax was rare. Conclusion The characteristic radiological changes in AIDS complicated with PSM patients were shown as nodules, nodules with cavities, miliary nodules, hilar and （or） mediastinal lymphadenopathy, hepatosplenomegaly and low density peritoneal lymphadenopathy.
