椎弓根复合体钉钩系统治疗青年腰椎峡部裂 被引量:12

Direct Repair of Lumbar Spondylolysis in Young Patients with Pedicle Screw Hook System Fixation

摘要 目的评价椎弓根复合体钉钩系统治疗青年腰椎峡部裂的临床疗效。方法选取2007年9月-2012年1月期间住院的28例青年腰椎峡部裂患者,手术采用椎弓根复合体钉钩系统进行治疗。术后随访13~63个月,临床结果采用视觉模拟评分(VAS)及Oswestry功能障碍指数(ODI)进行评估。结果 28例患者在术后3~12个月均恢复日常生活及工作,所有患者随访期间未见椎弓根钉及椎板钩断裂、松动或脱钩等并发症。CT检查峡部裂融合率为92.9%。术前及最后一次随访时VAS评分分别为(7.5±0.8)及(1.0±0.6),ODI指数为分别为(73.5±7.3)%及(10.6±4.0)%。结论椎弓根复合体钉钩系统直接修复法仅在伤椎上进行简单且符合正常生理解剖结构的复位固定,是一种有效的治疗青年腰椎峡部裂的手术方法,具有广泛的应用前景。 Objective To evaluate the clinical outcome of pedicle screw hook system fixation for direct repair of lumbar spon- dylolysis in young patients. Methods 28 young cases suffered from symptomatic lumbar spondylolysis from September 2007 to January 2012 were treated with pedicle screw hook system fixation. The average follow-up period was 39.1 months (range 13 -63 months). The clinical outcome was measured using visual analogue pain scale (VAS) and Oswestry disability index (ODI). Re- suits The preoperative VAS and ODI score ( % ) were decreased at the last follow-up, VAS (7.5 ± 0.8 ) vs. ( 1.0 ± 0.6 ) , ODI ( 73.5 ±7.3 ) % vs. ( 10.6 ± 4.0 ) %. No complications of screw or/and hook breakup, loosing or slide aside. The successful bone fusion rate was 92.9%. Conclusion Pedicle screw hook system fixation, as a direct repair method for lumbar spondylolysis, can reduce fusion segments, restore stability of the lumbar vertebrae, and is promising in the future.
