摘要
目的:研究心内直视手术术中排气引流方法,以减少术中溢血量,增加手术野清晰度,减轻血细胞的破坏。方法:将100例体外循环(CPB)心内直视手术患者随机分为观察组和对照组。观察组在开放升主动脉行心脏复跳时采用改良主动脉密闭引流排气法引流排气,对照组采用传统的开放式引流排气。观察两组开放升主动脉后CPB时间,开放升主动脉后第5、10、15、20、25、30min术野溢血量,血红蛋白尿发生率,术后气栓发生率。结果:观察组开放升主动脉后CPB时间明显短于对照组,差异具有统计学意义(P<0.01);开放升主动脉后各时间段术野溢血量明显少于对照组;血红蛋白尿发生率明显低于对照组,差异均具有统计学意义(P<0.01);术后两组均无明显气栓发生。结论:改良主动脉密闭引流排气法与传统的传统开放式引流排气相比较,具有缩短CPB时间,减少术野溢血量,增加术野清晰度,减轻对血细胞的破坏的优点,同时使用安全可行,术后无气栓发生,值得临床推广应用。
Objective： To explore a modified drainage method in open heart operation in order to get clear sight, reduce damage to blood cell and to decrease blood loss volume. Methods： A total of 100 patients scheduled for open heart operation under cardiopulmonary bypass （CPB） were divided randomly into two groups. For the observation group, modi[ied degassing with hermetic drainage was applied, while for the control group, traditional patent degassing was applied respectively during opening of the ascending a- orta to recover the heart beat. The CPB time and the suf{usion of operating field 5rain, 10rain, 15rain, 20rain, 25rain, 30rain after opening of the ascending aorta, the incidence of post-operational hematoglo- binuria and gas embolism were studied. Results： The CPB time after opening of the ascending aorta, thesuffusion of operating field in every time section, and the incidence of hematoglobinuria, of the observation group, were obviously shorter in the observation group （ P〈0 01）, as compared with the control group. No gas embolism was found in the two groups. Oonclusion.. Comparing with traditional patent degassing, modified degassing of the aorta with hermetic drainage may shorten the CPB time, decrease the suffusion and improve the legibility of operating field, extenuate the blood cell destruction. It＇s safe and worthappli- cation in large scale.
