营养支持对稳定期慢性阻塞性肺疾病患者肺功能及6min步行距离的影响 被引量:24

The effect of nutritional support therapy on lung function and 6 minute walk test in patients with stable chronic obstructive pulmonary disease

摘要 目的观察稳定期慢性阻塞性肺疾病(COPD)患者营养支持治疗对肺功能及6 min步行距离的影响。方法连续入组60例合并轻至中度营养不良的COPD患者,根据患者意愿分为营养组及对照组,营养组在常规治疗基础上加肠内营养混悬液(整蛋白纤维型)营养支持,对照组在常规治疗基础上加营养指导。观察营养支持治疗对肺功能及六分钟步行距离的疗效。结果营养组平均每日摄入热量(1 843.73±57.88)kCal,对照组平均每日摄入热量(1 087.53±51.16)kCal。治疗8 w后,营养组的营养指标、肺功能(FEV1/FVC、FEV1%预计值)和6 min步行距离均有改善,与对照组比均有统计学意义(P<0.05);对照组治疗后的营养指标、肺功能和6 min步行距离较治疗前稍有改善,但无统计学意义(P>0.05)。结论对于COPD患者,除常规治疗外,给予正确的营养支持治疗,可以改善COPD患者的营养状况、肺功能及6 min步行距离,对改善患者的生活质量有重要的意义。 Objective To observe the effect of nutritional support therapy on lung function and 6 minute walk test in patients with stable chronic obstructive pulmonary disease (COPD). Methods 60 COPD patients were recruited consecutively into nutritional support and control groups according to the patients' willing. Both groups were given standard COPD treatment according to GOLD 2011 guideline. Enteral Nutritional Suspension( TOTAL PROTEIN FIBRE) was given to nutritional therapy patients, whilst the control group was given nutritional advice on COPD therapy. The effect of nutritional support on lung function and 6 minute walk distance were analyzed. Reults The amount of heat of nutritional support group took in ( 1 843.73 ±57. 88) kCal per day, and took in ( 1 087.53 ±51.16) kCal per day in control group. After 8 w of treatment, lung function (FEV1/FVC ratio, FEVI% predicted) and 6 minute walk distance of nutritional support group had been improved statistically (P 〉 0.05) compared with those of control group. Conclusions For COPD patients, in addition to standard treatment, errrect nutritional support therapy might improve the nutritional status, lung function and 6 minute walk distance of COPD patients, which implies the improvements of the quality of life for COPD patients.
