摘要
目的探讨膜周部室间隔缺损(PMVSD)介入治疗后心律失常发生与血清肌钙蛋白Ⅰ(cTnⅠ)变化的相关性。方法对76例成功介入治疗的PMVSD患者,进行心电图和cTnⅠ同步检测,术前24 h动态心电图检查和术后72 h心电监护,术后随访并进行临床和心电图检查和心脏彩超检查。结果介入治疗72 h内,心律失常发生率32.89%(25/76),术后cTnⅠ升高的峰值在术后4 h,心律失常发生率的峰值在术后12 h,术后4 h cTnⅠ的均值和术后12 h心律失常发生率呈正相关(r=0.90,P<0.05)。结论 1)PMVSD的介入治疗有效,且介入治疗的相关心律失常安全性较高,无严重不可逆并发症发生;2)PMVSD介入治疗后心律失常发生与cTnⅠ的变化有关,cTnⅠ变化对心律失常的发生和严重程度可能有预测价值。
Objective To study the relationship between arrhythmia and cardiac troponin I （cTn I） after interventional therapy for perimembranous ventricular septal defects（PMVSD）. Methods The pr- eoperative 24-hour dynamic electrocardiographic examination before operation and postoperative 72- hour electrocardiographic monitoring were performed and cTn I levels were synchronously detected in 76 patients who underwent successful interventional treatment for PMVSD. The follow-up was carried out and electrocardiographic and echocardiographic examinations were performed in all patients. Results Within 72 hours after interventional therapy, the incidence of arrhythmia was 32.89% （25/76）. The peak concentrations of cTn I and peak incidence of arrhythmia, respectively, appeared at 4 and 12 hours postoperatively. There was a positive correlation between mean cTn I concentrations and arrhyt- hmia incidence （r=0.90,P〈0.05）.Conclusion The interventional therapy is effective and safe for PMVSD, without serious irreversible complications. After interventional therapy,the incidence of arrhythmia is correlated with the changes in eTn I concentrations. Therefore, cTn I detection may be used to predict the occurrence and severity of arrhvthmia.
