摘要
目的探讨妇科腹腔镜手术相关并发症的发生原因、处理及预防措施。方法对我院2010年1月～2011年12月的3076例妇科腹腔镜手术的临床资料进行回顾性分析。结果 3076例手术中,腹腔镜手术相关并发症17例,发生率0.55%,其中阴道残端愈合不良3例,切口疝、腹壁切口出血、膀胱损伤、肌瘤残留各1例需要再次手术;3例出血较多术中转开腹处理;阴道残端愈合不良、持续性异位妊娠各2例,子宫穿孔、输尿管损伤、切口感染各1例经保守治疗治愈,无死亡病例。结论严格把握腹腔镜手术的适应证,认真、谨慎开展符合自己水平的手术,提高手术医师的操作技能,必要时中转开腹是减少并发症发生的关键。
Objective To investigate the causes of gynecological laparoscopic surgery-related complications, and their management and preventive measures. Methods A retrospective analysis was carried out in 3076 cases of gynecological laparoscopic surgery in our hospital from January 2010 to December 2011. Results In the 3076 cases, laparoscopic surgery-related complications occurred in 17 cases （0.55%） , of which three cases showed unhealed vaginal stump, and other complications including incision hernia, abdominal incisional hemorrhage, bladder injury, fibroids residual was detected in one patient respectively, three cases of massive hemorrhage was converted to laparotomy. Two patients with unhealed vaginal stump, two patients with recurrent ectopic pregnancy, one with uterus perforation, one with ureteral injury, and one with incisional infection were healed. No patient died. Conclusion Strict control of the surgical indications for laparoscopic surgery, and careful operation, efforts in improving surgical skills and reducing rate of conversion to laparotomy when necessary are the keys to reducing complications.
