摘要
[目的]构建成人心胸外科术后病人手法振动按压法协助排痰效能评价体系,比较手法振动按压法与手法振动法协助排痰的有效性,探索影响心胸外科术后病人术后排痰效能的因素。[方法]以ADC效能评估理论为概念框架,通过德尔菲专家咨询构建指标体系,采用随机抽样的方法将184例病人分为干预组和对照组,干预组采用手法振动按压法,对照组采用手法振动法,比较两组排痰效能及并发症发生情况,并分析影响排痰效能的因素。[结果]构建的成人心胸外科术后病人手法振动按压法协助排痰效能评价体系包括3个维度、11条评价指标、42条评价条目,内容效度为0.882,0.891,具有良好的内部一致性和时间稳定性,函询专家的可靠性和权威程度较高;手法振动按压法排痰效能优于手法振动法,影响排痰效能的因素主要有人口学特征、疾病特征、手术管理、肺部状况、出入量及胸腔积液量。[结论]成人心胸外科术后病人手法振动按压法协助排痰效能评价体系具有良好的内部一致性和时间稳定性,权威性较高,且手法振动按压效果优于手法振动法,影响心胸外科病人术后排痰效能的因素较多,应采取各种有利于排痰因素的措施。
Objective：To construct the performance evaluation system of manual vibration pressing method assisting expectoration for adult patients after undergoing cardiothoracic surgery and to compare the effectiveness between manual vibration pressing method and manual vibration method assisting expectoration and to explore its influencing factors. Methods： Based on ADC effectiveness assessment theory as concept famewok,through the Delphi expert advice to construct the index system,a total of 184 cases of postoperative cardiothoracic patients were divided into control group and intervention group by using random sampling. Intervention group patients used manual vibration pressing method,the control group patients used manual vibration method, then to compare the expectoration effect and complications, and to analyze the influencing factors of expectoration effect. Results：Constructed perform- ance evaluation system of manual vibration pressure method assisting sputum expectoration included 3 dimensions, 11 evaluation indexes and 42 evaluation items. The content validity was 0. 882,0. 891. It has good internal consistency and time stability. And it has good experts＇ reliability and authority ; The efficiency of manual vibration pressure method was better than that of manual vibration method in assisting sputum expectoration. The influencing factors of expectoration included different demographic characteristics, disease characteristics, operation management, lung condition,intake and output, and the amount of pleural fluid. Conclusion：The performance evaluation system of manual vibration pressing method assisting expectoration for adult patients after undergoing cardiothoracic surgery has good internal consistency and time stability, has higher authority. Manual vibration pressing is better than manual vibration method, and influencing factors of expectoration performance are multiple. One should take conducive measures for expectoration.
