摘要
目的:应用彩色多普勒超声检测妊娠高血压疾病重度子痫前期孕妇药物治疗前后肾动脉血流的参数变化特点,评价其临床应用价值。方法:以正常孕妇100例为对照组,选取经临床确诊的30例重度子痫前期孕妇作为治疗前组,以该病例组药物治疗后作为治疗后组。采用SEQUOIA 512型超声诊断仪、阿洛卡a5型超声诊断仪,探头频率3.5~5.0 MHz,观察各组肾内结构,应用脉冲多普勒测量各组孕妇肾叶间动脉(IRA)收缩期血流峰值流速(PSV)、舒张末期血流流速(EDV)、加速时间(AT)、阻力指数(RI)、PSV/EDV(S/D)。结果:治疗前组PSV(28.72±1.21)m/s、EDV(6.78±2.13)m/s明显低于对照组和治疗后组;RI(0.75±0.05),AT(0.10±0.04)s,S/D(3.76±0.22)明显高于对照组和治疗后组,差异有统计学意义(P<0.05)。结论:多普勒超声检查评价重度子痫前期孕妇药物治疗前后肾动脉血流参数变化,重复性强,为临床诊断和治疗重度子痫提供了重要的依据,具有较高的临床应用价值。
Objective: To investigate the characteristics of the parameter changes of renal artery blood flow before and after drug treatment in pregnancy-induced hypertension and severe eclampsism pregnant women by color doppler ultrasonography, and evaluate its clinical application value. Methods: 100 normal pregnant women were selected as the control group. 30 cases of severe eclampsism pregnant women were selected as pretherapy group. The same 30 women in pretherapy group after medical treatment were taken as post-treatment group. Ultrasonography studies were performed with Acuson Sequoia 512 and ALOKA c^5 equipment, probe frequency 3.5-5.0 MHz. Observed the inner structures of kidneys of all gravida. Measured the peak systolic velocity (PSV), end diastolic velocity (EDV), acceleration time (AT), resistance index (RI), peak systolic velocity and end- diastolic maximum velocity ratio (S/D) of interlobar renal artery (IRA) of all kidneys by pulsed Doppler. Results: PSV and EDV of pretherapy group were (28.72 _+ 1.21) m/s, (6.78 ~ 2.13) m/s respectively. These were signifi- cantly lower than those of the control group and post-treatment group. RI (0.75 + 0.05). AT (0.10 ~ 0.04) s and S/D (3.76 ~ 0.22) of pretherapy group were significantly higher than those of the control group and post- treatment group (P 〈 0.05). Conclusion: Doppler ultrasound can detect the changes of renal artery blood flow of severe eclampsism pregnant women before and after drug treatment, the repeatability is good. It can provide important evidence for clinical diagnosis and treatment of severe eclampsia, which has high clinic value.
