实时声学造影结合多巴酚丁胺负荷评价存活心肌的临床研究 被引量:1

摘要 目的:评价实时心肌声学造影(RT-MCE)联合多巴酚丁胺负荷检测存活心肌的临床价值。方法:43例患者心脏彩色多普勒超声显示左心室室壁运动减弱,同时能行血运重建术的共获得237个心肌节段。血运重建前3天内行心肌声学造影,观测静脉滴注多巴酚丁胺前后室壁运动区域心肌微循环造影剂充盈增强程度,改善≥1分认为存在存活心肌;血运重建术后3个月复查心脏超声同术前做比较,室壁运动改善≥1分判断存在存活心肌。结果:冠脉血运重建术3个月后,室壁运动改善有155个节段,无改善有82个节段。MCE评价有存活心肌为158个节段,无存活心肌79个节段,敏感性、特异性及准确率分别为91.6%、80.5%、87.7%。结论:MCE联合药物负荷能较准确的预测存活心肌,存活心肌的检测有助于鉴别哪些冠心病患者冠状动脉再血管化后收益,优化治疗方案,指导临床诊疗决策具有重要意义。
