摘要
目的:探讨四种肿瘤标志物癌胚抗原(CEA)、糖类抗原(CA12-5)、神经元特异性烯醇化酶(NSE)、细胞角蛋白19片段(CYFRA21-1)联合应用在肺癌诊断中的临床价值。方法:采用电化学发光法检测189例肺部良性病患者、100例经病理确诊肺癌患者、60例健康人的血清CEA、CA125、NSE、CYFRA21-1的水平及阳性率,对比分析四种肿瘤标志物联合及单独应用的敏感度、特异度、准确度、阳性预测值、阴性预测值、阳性似然比、阴性似然比等。结果:肺癌组CEA、CA125、NSE、CYFRA21-1水平均明显高于良性病变组(P<0.05)及健康对照组(P<0.01)。结论:四项联合检测可提高肺癌诊断的敏感度和准确度,优于其它单项检测和其它联合检测,具有较高的临床应用价值。
Objective Discuss so study the clinical value of four kinds of tumor markers including carcinoembryonic antigen（CEA）,car- bohydrate antigen（CA12-5）,neuron specific enlace（NSE）,cytokeratin 19 fragment（CYFRA21-1）combined detection in diagnosis of lung cancer. Methods Using electrochemical luminescence method detection in 189 patients with lung benign disease patients,100 cases of pathologically confirmed lung cancer patients,60 healthy persons＇serum CEA,CA125,NSE,CYFRA21-1 levels and positive rates,com- parative analysis of four kinds of tumor markers＇joint and separate application of sensitivity,specificity,accuracy,positive predictive value, negative predictive value,positive likelihood ratio,negative likelihood ratio and soon on. Results The CEA,CA125,NSE,CYFRA21-1 lev- els in lung cancer group were significantly higher than that in benign disease group （P〈0.05）and healthy control groups （P〈0.01）. Conclusion Joint detection can enhance the sensitivity and accuracy lung cancer diagnostic,better than other individual testing and other joint detection,it has high value in clinical application.
