摘要
目的探讨抑郁症患者前额叶、海马氢质子磁共振波谱(1H-MRS)特点。方法以46例抑郁症患者为研究对象(病例组),以同期46名正常人为对照组,采用多体素1H-MRS在2组对象入组24 h内检测双侧前额叶和海马N-乙酰基天门冬氨酸(NAA)、胆碱复合物(Cho)与肌酸复合物(Cr),并计算NAA/Cr和Cho/Cr值。结果病例组左侧和右侧前额叶、左侧海马NAA/Cr值分别为1.77±0.18、1.78±0.23和1.75±0.21,低于对照组的1.87±0.16、1.89±0.15和1.87±0.27,差异均有统计学意义(t=2.816,2.717,2.379,P<0.01或0.05);右侧海马NAA/Cr值与对照组比较,差异无统计学意义(P>0.05)。海马NAA/Cr值单侧化指数与1.00相比,对照组差异有统计学意义(P<0.01),病例组差异无统计学意义(P>0.05)。结论抑郁症患者存在双侧前额叶及左侧海马神经元的结构和(或)功能异常,其海马不对称性消失。
Objective To identify the characteristic of proton magnetic resonance spectroscopy(1H-MRS) on prefrontal lobe and hippocampus in patients with depressive disorder.Methods Forty-six patients with depression(depression group) and 46 normal controls(control group) were assessed in this study.The N-acetylaspartate(NAA),choline-congtaining compounds(Cho) and creatine compounds(Cr) in bilateral prefrontal lobe and hippocampus were detected by multi-voxel 1H-MRS with 24 hours,then the values of NAA/Cr and Cho/Cr were computed.Results The value of NAA/Cr in left prefrontal lobe,right prefrontal lobe and left hippocampus in the depression group was 1.77±0.18,1.78±0.23 and 1.75±0.21,respectively,while the value in the control group was 1.87±0.16,1.89±0.15 and 1.87±0.27,respectively.The values in depression group were lower than those in control group(t=2.816,2.717,2.379;P0.01 or 0.05).There was no statistically significant difference in the value of right NAA/Cr between the two groups(P0.05).Compared with 1.00,the lateralization index of NAA/Cr in hippocampus had significant difference in control group(P0.01),but had no significant difference in depression group(P0.05).Conclusion The neuronic structures and/or function in bilateral prefrontal lobe and left hippocampus are anomalous in patients with depressive disorder,the lateralized asymmetry of hippocampus disappear.
