摘要
目的探讨北京市顺义区应用急救医疗服务(EMS)救治急性心肌梗死(AMI)的状况以及影响患者应用EMS延迟的因素,并提出避免延迟的策略。方法采用回顾性研究方法,分析顺义区120自2008年1月-2010年12月救治转运的38691例中824例AMI患者应用EMS的情况以及延迟应用的原因。结果824例AMI患者中,因各种原因延迟接受EMs服务的685例,占83.1%,一级、二级医院转诊延迟发生分别为102、322例,分别占14.9%、47%。发生并发症患者508例,占61.7%,猝死198例占24%。结论北京市顺义地区人们的急救意识还很淡漠,就诊延迟率高,需加强急救知识的宣传,并建立AMI患者的绿色通道,急救中心建立指挥调度中心。各级医院严格执行上报制度,协调好AMI患者的接转诊。
Objective To investigate the current situation of application of the emergency medical services (EMS) among the acute myocardial infarction (AMI) patients in Shunyi District, Beijing, and the influential factors of delay in service delivery. Methods The relevant materials of the 824 AMI patients out of the 38 691 patients who were transported to appropriate hospitals by the ambulances of the 120 EMS system during the period from January 2008 to December 2010 in Shunyi District were analyzed. Results 39.1% of the 824 patients (n=322) were transferred by the grade 1 or grade 2 hospitals due to shortage of ward bed, and the family members of 198 patients(24.0%) had definite first aid knowledge and called 120 in time. However, 685 of the 824 patients failed to receive timely treatment of the 120 EMS because the ambulances came late. Delay of referral occurred in 102 and 322 cases by grade 1 and grade 2 hospitals respectively. Complications occurred in 508 patients (61.7%), including 198 cases of sudden death (24%). Conclusion The rate of delayed treatment in the EMS system of Shunyi District is still rather high. The awareness rate of AMI among the people there is to be raised.
