摘要
目的 探讨与氯氮平对慢性难治性精神分裂症疗效有关的 5 羟色胺 2A(5 HT2A)受体基因的基因型及其他相关因素。方法 抽取 10 4例慢性难治性精神分裂症患者 ,给予氯氮平≥ 40 0mg/d治疗 2个月。治疗前后用阴性和阳性症状量表 (PANSS)评定氯氮平疗效 ,按PANSS的减分率≥ 30 %和 <30 %将氯氮平治疗的患者分为有效组和无效组 ,用聚合酶链反应扩增及限制性片段长度多态性(PCR RFLPs)技术测定患者的基因型和等位基因。所得资料进行 χ2 、t检验及Logistic回归分析。结果 氯氮平对慢性难治性精神分裂症患者的疗效与男性患者呈正相关 (P <0 0 1,RR =3 44 ) ,与 5 HT2A受体基因的基因型A1/ 1呈负相关 (P <0 0 5 ,RR =0 2 8)。结论 氯氮平对慢性难治性精神分裂症患者的疗效受性别影响 ,5
Objective To investigate the relationship between the responses to clozapine and the 5 hydroxytryptamine type 2A (5 HT 2A ) receptor gene and the other related factors in chronic refractory schizophrenic patients Methods 104 chronic treatment refractory in patients with schizophrenia had been treated by clozapine (≥400 mg/d) for 2 months The responses to clozapine were rated with Positive and Negative Symptom Scale (PANSS). The patients were divided into two groups according to the reducing rates of PANSS (≥30% and <30%, respectively) The genotypes and alleles in these patients were examined with Restriction Fragment Length Polymorphism (RFLPs) technique The significance for the associations was estimated by the χ 2 and t test and the logistic regression analysis Results The responses to clozapine in the patients are positively related to the male ( P <0 01, RR =3 44) and negatively related to the A 1/1 genotype of the 5 HT 2A receptor gene ( P <0 05, RR =0 28) Conclusions The responses to clozapine in chronic refractory schizophrenic patients are influenced by the sexes The 5 HT 2A receptor gene may play a role in the action of clozapine in the chronic refractory schizophrenic patients
