摘要
目的探讨肩周炎患者肩关节囊的超声生理测值及声像图特征,为肩周炎患者的临床诊断提供客观依据。方法对120例健康体检者(正常组)与40例肩周炎患者(肩周炎组)的超声生理测值及声像图特征进行对比分析。结果 (1)正常组120例肩关节囊下壁显示满意率为100%,男女、左右侧厚度无明显差异(P>0.05)。(2)肩周炎组患者肩关节囊下壁显示满意率为75%,与肩周炎病变致前臂外展、上举受限程度明显相关。与正常组厚度测值比较,肩周炎组病变侧关节囊明显增厚(P<0.05)。(3)正常组超声显示关节囊为走行清晰、分布均匀的低回声及其外侧细带状高回声结构;肩周炎组超声声像图显示肩关节囊下壁增厚毛糙,结构不清,低回声带明显水肿。结论肩关节囊壁厚度的超声检查及生理测值量化指标具有实用价值,高频超声对肩周炎诊断有重要意义。
Objective To evaluate the ultrasound physiological measurement and image feature of scapulohumeral periarthritis patients,to provide the objective evidence for its clinical diagnosis.Methods 120 healthy volunteers(normal group) and 40 scapulohumeral periarthritis patients(scapulohumeral periarthritis group),whose ultrasound physiological measurement and ultrasonographic images characteristic were contrasted.Results(1) 100% of 120 normal adult got clear images of their shoulder joint capsule at the site of anterior.There were no statistical significance between male and female and the left-and right-side of the shoulder(P0.05).(2) 75% of scapulohumeral periarthritis patients got images of their shoulder joint capsule,display rate was highly correlated with degree of forearm abduction and superduct confined by scapulohumeral periarthritis.The thicknesses of patient group were significantly thicker then the normal group,there were significant differences(P0.05).(3)The structures of shoulder joint capsule were visualized clearly on the healthy adult shoulder ultrasonographic images,they were with homogeneous echo and hyperechoic line outside it.Ultrasonography of scapulohumeral periarthritis group were thicken joint capsule with vague boundary,diffuse inhomogeneous internal echo,hypoechoic line swelling,edema and adhesion.Conclusion The quantization ranges of normal values and ultrasonography of shoulder joint capsule would be useful in clinical practice.High-frequency ultrasound has significance for the diagnosis of scapulohumeral periarthritis.
