摘要
目的评价采用国际内固定学会(AO/ASIF)治疗股骨中上段长节段骨折治疗方法的临床疗效。方法 2001年3月～2011年2月,我院采用AO/ASIF内固定治疗技术对股骨中上段长节段骨折76例进行诊治,其中采用加长型PFNA技术12例,DHS和/或DCS治疗32例,PFN治疗19例,加长股骨近端锁定板治疗13例。76例患者均为股骨闭合性骨折,术后第1、3、6、9、12个月门诊随访,以后每年至少门诊复查1次。结果 76例患者均获得完整随访。术后随访1～21个月,平均15个月,X线片显示所有患者均骨性愈合,无断钉、退钉及股骨头切割现象。按Harris髋关节功能评分标准,优55例,良14例,中5例。结论 AO/ASIF加长型PFNA治疗方法具有微创、固定强度高、生物力学好等优点,在治疗股骨中上段长节段骨折时骨愈合高,功能恢复快,并发症少,宜于首选。
Objective To evaluate the clinical efficacy of treatment using international internal fixation society （AO/ASIF） treatment began long segments of the femoral fractures. Methods From March 2001 to February 2011,with the use of AO/ ASIF internal fixation techniques our department have accomplished 76 cases of upper middle long segments of the femoral fracture treatment. Among them,there were 12 examples of the accumulation PFNA, 32 cases of DHS and DCS treatment, 19 cases of PFN treatment,and the rest 13 cases were treated with the extended proximal femur lockplate treatment. After surgery there continued to be follow-up visits of outpatient service in the lst,3rd,6th,9th and 12th months.Afterwards we had each patient checked in the outpatient service at least once a year. Results A total of 76 patients obtained complete follow-ups. Follow-ups varied from 1 to 21 months, on average, 15 months, and X-ray showed all of the patients had osseous healing, no screw breakage, refund nails,or cutting phenomenon on the femoral head. According to the Harris hip functional score standard：it turned out to be 53 excellent cases,13 good cases and 5 medium cases. Conclusion AO/ASIF axtended PFNA treatment method reflected its minimally invasiveness,high fixe strength,excellent biomechanics, and in the treatment of upper middle long segments of the femoral it contributed to high level of fracture bone healing,fast function recovery,less complications and should be the first choice.
