2型糖尿病患者血糖、胰岛素抵抗和β细胞功能对CA199水平的影响 被引量:14

The effects of blood glucose,insulin resistance and β-cell function on CA199 level in patients with type 2 diabetes mellitus

摘要 目的探讨T2DM患者CA199水平与血糖、IR和胰岛β细胞功能之间的关系。方法入选T2DM患者165例,分别以HbA1c 10.1%、FPG 8.31mmol/L、胰岛素抵抗指数(HOMA-IR)2.7、胰岛功能指数(HOMA-islet)22.1为界,分为高HbA1c组(HH组)和低HbA1c组(LH组);高FPG组(HF组)和低FPG组(LF组);IR组和胰岛素敏感组(SR组);胰岛β细胞功能正常组(BN组)和胰岛β细胞功能减退组(BW组),比较各组CA199水平。结果 HH组CA199水平高于LH组(26.75 vs 16.68U/ml,P<0.05)。Spearman相关研究显示,CA199与HbA1c、FPG呈正相关(P<0.05),与IR、胰岛β细胞功能无相关性(P>0.05);多元逐步回归分析显示,HbA1c是CA199的主要影响因素。结论 T2DM患者CA199水平与血糖状态密切相关。 Objective To investigate the relations of CA199 level with blood glucose, insulin resistance, and β cell function in patients with type 2 diabetes mellitus. Methods The 165 hospitalized patients with type 2 diabetes were enrolled. They were divided into high glycosylated hemoglobin group (HH) and low glycosylated hemoglobin group (LH) at the cut level of HbA1 c 10. 1 %, into high fasting blood glucose group (HF) and low fasting blood glucose group (LF) at the cut level of FPG 8. 31 mmol/L, into insulin resistance group (IR) and insulin sensitivity group (SR) at the cut level of HOMA- IR 2. 7, and into normal β cell function group (BN) and low β cell function group (BW) at the cut level of HOMA-β 22.1. The CA199 level among the four groups was compared. Results The CA199 level of the HH group at 26. 75 U/ml was significantly higher than the LH group at 16. 68 U/ml (P〈0. 05). Spearman relation analysis showed that the CA199 level was positively correlates with HbA1 c and FPG (P〈0. 05), but not correlated with IR and 13 cell function (P〉0. 05). Multiple stepwise regression analysis showed that HbA1 c was the main risk factor for CA199. Conclusion The CA199 level in patients with type 2 diabetes closely correlates with the state of blood glucose.
