摘要
目的比较CEUS三维重建(3D-CEUS)与三维彩色血管能量成像(3D-CPA)显示肾脏肿瘤血管结构的价值。方法收集经病理证实的肾脏肿瘤患者121例,依次行常规超声、CEUS、3D-CPA、3D-CEUS检查并存储图像。应用QLab软件进行三维图像重建,按血供丰富程度将图像分为4级。比较3D-CEUS与3D-CPA对肾脏肿瘤血管结构的显示情况。结果在CEUS不同时相,3D-CEUS对肾脏良、恶性肿瘤血管结构的显示差异均有统计学意义(P均<0.05),而3D-CPA对肾脏良、恶性肿瘤血管结构的显示差异无统计学意义(P>0.05)。恶性肾脏肿瘤主要表现为血流丰富、血管纡曲扩张呈粗网状或杂乱树枝状。良性肾脏肿瘤主要表现为血供较少,呈云雾状或云雾状叠加网状。3D-CPA与3D-CEUS在实质早期有相关性(P<0.05)。结论三维超声成像可立体显示肾脏肿瘤血管结构,3D-CEUS显示肾脏肿瘤血管结构优于3D-CPA。
Objective To compare the value of three-dimensional CEUS (3D-CEUS) and three-dimensional color power angiography (3D-CPA) in evaluating blood supply of renal tumors. Methods One hundred and twenty-one patients with renal tumors confirmed by pathology underwent 3D-CEUS and 3D-CPA after conventional ultrasound and CEUS. Analysis was performed using QLab software. Renal tumors were categorized into 4 grades based on the blood supply assessed by 3D-CEUS or 3D-CPA. Blood supply of renal tumors was compared between 3D-CEUS and 3D-CPA. Results Benign and malignant renal tumors showed significant differences in blood supply in the different phases of 3D-CEUS (all P〈0.05), but not of 3D-CPA (P〈0.05). Malignant renal tumors showed abundant blood flow, tortuous expansive reticulation or ir- regular messy dendritic arb.orization, while benign renal tumors showed less blood flow, nebulous or reticular on the basis of nebulous. Moreover, blood supply of renal tumors during early parenchymal phase assessed by 3D-CEUS correlated well with 3D-CPA (P〈0.05). Conclusion Three-dimensional ultrasonography can depict clear stereoscopic structures and morphological characteristics of renal tumors. 3D-CEUS is superior to 3D-CPA in assessment of blood supply of renal tumors.
