摘要
目的:更昔洛韦眼用凝胶和阿昔洛韦滴眼液治疗单纯疱疹性角膜炎的疗效比较。方法:128例患者随机分成对照组和观察组,观察组用0.15%更昔洛韦眼用凝胶局部滴患眼,对照组用0.1%阿昔洛韦滴眼液局部滴患眼,2周后复诊,通过记录患者的眼部刺激症状,球结膜睫状充血程度,角膜改善情况来比较两种药物的疗效。水平分析两者治疗效果的差异。结果:观察组64眼,治愈34眼,治愈率约53.0%;,好转30眼,总有效率100.0%。对照组64眼,治愈12眼,治愈率约19.0%;好转28眼,无效24眼,总有效率62.5%。观察组未发现不良反应。对照组有4例用药后眼部疼痛,但可忍受,4例用药后球结膜睫状充血一过性加重,都可继续用药。观察组疗效明显优于对照组。对治愈者随诊12月,观察组34例治愈者复发2例,复发率约6.0%。对照组12例治愈者复发8例,复发率约67.0%。观察组复发率明显低于对照组。两组治疗结果从治愈率,总有效率,复发率,不良反应几方面对比有显著差异(P<0.05)。结论:更昔洛韦眼用凝胶与阿昔洛韦滴眼液相比,具有疗效显著,作用持久,副作用低,治疗后不易复发等优点,是目前治疗HSK的较理想的药物之一,值得临床应用。
Objective： To compare the effects of ganciclovir eye gel and acyclovir eye drops on simple herpes keratitis. Methods： A total of 128 patients were randomly divided into control group and observation group, the observation group was treated with topical application of 0. 15% ganciclovir eye gel, while the control group was given 0. 1% topical acyclovir eye drops. After 2 weeks of treatment, status such as eye irritation, bulbar conjunctiva ciliary hyperemia and improvement of cornea symptoms were re- corded and compared between the two groups. Results： Of the 64 eyes of the treatment group 34 were healed, with and curative rate of 53.0 %, 30 showed improvements indicating a total efficiency of 100.0 %. For the control group, 12 of 64 eyes were cured （19.0%） ; 28 were improved, and 24 showed no improve- ment, the total efficiency of the control group was 62. 5%. No side effects was observed in the observation group, but ocular pain which was tolerable was observed in 4 cases of the control group, ciliary congestion of the bulbar conjunctiva after treatment occurred in other 4 cases. According to follow up of 12 months, 2 （6.0K）and 8 cases （67.0%） of recurrence were observed in the observation group and control group, respectively. Significant difference in curative rate, total efficiency rate, recurrence rate and incidence of side effects were observed （P〈0.05）. Oonclusions. Ganciclovir ophthalmic gel is more effective on simple herpes keratitis than acyclovir eye drops, it produce longer term efficiency but induce lower side effects, and lower recurrence rate, thus it＇s an optimum treatment for herpes simple keratitisand and is worthy of clinical aot）lication.
