摘要
目的:观察健脾理气方(自制)鼻饲联合按揉足三里、关元穴位对肠源性脓毒症的临床疗效观察。方法:将43例肠源性脓毒症患者随机分为两组,其中治疗组23例及对照组20例,对照组予常规治疗,治疗组在常规治疗基础上鼻饲健脾理气中药协定方,每次100mL,服14天;同时采用指按法和指揉法按揉足三里、关元穴位,每穴5～10min,每日2次,连续14天。观察两组患者治疗前后①肠鸣音和腹部症状、体征的改善情况;②胃肠功能评分;③检测血中C反应蛋白(CRP)、降钙素原(PCT)及D-乳酸的变化;④治疗前后APACHEII评分和机体炎症反应严重程度的评分(SIRS评分)改善情况;⑤记录不良反应发生情况。结果:治疗后治疗组总有效率明显高于对照组(P<0.05);治疗组CRP、PCT、D-乳酸的变化明显优于对照组(P<0.05);治疗组胃肠功能评分、APACHEII评分、SIRS评分明显优于对照组(P<0.05)。两组均未见严重不良反应。结论:健脾理气方配合按揉足三里、关元穴位对肠源性脓毒症有明显疗效。
Objective： To evaluate the clinical effect of Chinese medicine combined with acupoint massage at Zusanli （ ST36）, Guanyuan（CV4） in the therapy of gut - derived sepsis. Methods ： 43 cases of gut - derived sepsis were randomly divided into two groups, including treatment group with 23 cases and control group with 20 cases. 20 patients in the con- trol group were given conventional therapy. 23 patients in the treatment group were given conventional therapy combined with additional treatment for 14 days. The additional treatment included Chinese medicine 100mL/day and acupoint mas- sage Zusanli （ST36）, Guanyuan（CV4） 5 ~ 10min, twice a day. The following items were observed before and after treat- ment： bowel sounds, abdominal symptoms and signs of improvement; gastrointestinal function score; changes of serum C - reactive protein（CRP） ,procalcitonin（PCT） and D -lactic acid;APACHEII score before and after treatment and the se- verity of the inflammatory response score（ SIRS score）and adverse reaction. Results ：After therapy,the therapeutic effect in the treatment group was significantly better than that in control group（ P 〈 0.05）. Changes of CRP, PCT and D - lac- tic acid in treatment group was better than those in control group （ P 〈 0.05）. The gastrointestinal function score, APA- CHEII score and SIRS score in treatment group were significantly better than those in the control group （ P 〈 0.05）. No serious adverse reaction occurred in both two groups. Conclusion： Chinese medicine combined with acupoint massage has significant effect on the therapy of gut - derived sepsis.
