摘要
目的了解脂血对临床生化常用检验指标的干扰:TG、Urea、ALT、AST、GLU、PA、TCHO、HDL、LDL,比较消除干扰方法的临床意义。方法采用乳糜干扰物和高甘油三酯混合血清分别制作系列浓度干扰标本,在离心和稀释前后检测上述生化指标,结果行偏差计算和线性回归统计。结果高TG混合血清干扰物TG=5.75mmol/L:两种处理方法 ALT、AST、LDL均在允许误差范围(Ea)内;TG=13.87mmol/L:高速离心法PA,HDL在Ea内;TG=16.56mmol/L:稀释法PA、HDL在Ea内;TG、Urea、AST、GLU、TCHO、HDL、LDL经高速离心相关系数r>0.98,TG、Urea、ALT、AST、GLU、TCHO经稀释r>0.98。乳糜干扰物TG=13.10mmol/L:ALT、AST、HDL、LDL三种处理方法均在Ea内,UREA经高速离心+稀释法在Ea内,TG=14.53mmol/L时PA经高速离心在Ea内;GLU经高速离心r=0.98,TCHO经高速离心=0.99和高速离心+稀释r=0.99,HDL高速离心+稀释r=1.0。结论消除脂血的干扰方法中高速离心法优于稀释法,重度乳糜标本可通过高速离心法结合稀释法提高检测结果的准确性。
Objective To know lipids blood' s interference in clinical biochemical tests common indexes, including tri- glyceride (TG), urea (Urea), alanine aminotransferase (ALT), aspartate aminotransferase (AST), glucose (GLU), prealbumin (PA) and total cholesterol (TCHO), high density lipoprotein cholesterol ( HDL ), low density lipoprotein cholesterol ( LDL ), and to compare the clinical significances of eliminating interference methods. Methods Chyle disruptors and high triglyceride mixed serum specimens were applied to prepare a series of concentration disturbances in specimens. The above biochemical index were dedected before and after centrifugation and dilution by automatic biochemical analyzer, and the results were dealed with by deviation calculation and linear regression statistically. Results When high TG mixed ser- um disruptors was TG=5.75mmol / L, ALT, AST, LDL were in the error allowable range (Ea) using two treatment methods. WhenTG=13.87mmol/L, PA, HDL were in Ea by high speed centrifugation. When TG=16.56mmol/L, PA, HDL were Ea by dilution. When Chyle disruptors was TG=13.10mmol/L, ALT, AST, HDL, LDL were within Ea by three processing methods. UREA was in Ea by high speed centrifugation and dilution. When Chyle disruptors was TG=14.53mmol/ L, PA is in Ea by high speed centrifugation. High TG mixed serum disruptors by high speed centrifugation method TG, Urea, AST, GLU, TCHO, HDL, LDL could obtain more accurate results related coefficient r 〉0.98. Also TG, Urea, ALT, AST, GLU, TCHO could obtain more accurate results using dilution method (r〉0.98). Chyle disruptors could obtain more accurate results: GLU by high speed centrifugation (r =0.98), TCHO by high speed centrifugation (r=0.99) and high speed centrifugal + dilution (r =0.99), HDL high speed centrifugal + dilution ( r =1). Conclusion High-speed centrifugal method is better than the dilution method in disturbance methods of eliminating blood lipids. It can be improve the accuracy of the test results by high speed centrifugation combined with dilution method with severe chylous specimens.
