子宫内膜癌DWIBS定量测量及与PCNA表达情况的相关性研究 被引量:1

Correlative Study between Quantitative Measurement of Diffusion Weighted Imaging with Background Suppression and PCNA in Endometrial Cancer

摘要 目的 利用背景抑制弥散加权成像(DWIBS)定量测量子宫内膜癌肿瘤组织的信号强度和表观弥散系数(ADC)值,探讨肿瘤组织ADC值与增殖细胞核抗原(PCNA)表达情况的相关性.方法 收集具有完整磁共振资料和临床资料并经病理学证实的子宫内膜癌患者33例.分别行女性盆腔常规磁共振及DWIBS检查,测量子宫内膜癌肿瘤组织的信号强度和ADC值,并对不同病理分级子宫内膜癌肿瘤组织的信号强度、ADC值进行分析;免疫组织化学法测定PCNA表达情况,并与肿瘤组织ADC值进行相关性分析.结果子宫内膜癌肿瘤组织的平均信号强度和ADC值分别为(172.37±14.12),(0.94±0.17) mm2/s;高、中分化子宫内膜癌肿瘤组织的ADC值之间存在统计学差异(P<0.05),但信号强度之间无统计学差异(P>0.05):子宫内膜癌肿瘤组织的ADC值与其PCNA表达情况呈负相关(r=-0.81,P <0.05).结论 子宫内膜癌肿瘤组织ADC值定量测量对其病理分级有重要临床价值,同时可在一定程度上提示肿瘤组织的PCNA表达情况. Objective Quantitative measure the signal intensity and ADC values of the tumor tissue of endometrial cancer by DWIBS,and explored the relationship between the ADC values of tumor tissue and the ethological indicators of PCNA. Methods Collected 33 cases of endometrial cancer with integrated MRI data and clinical data,which diagnosed for the first time in our hospital and pro,,,ed by pathology. Female pelvic conventional MRI and DWIBS examination were inspected,to measure the signal intensity and ADC values of tumor tissue of endomentrial cancer and analyze the difference among different histologic grade. PCNA expression in tumor tissue of endometrial carcinoma were detected by immunohistoehemieal method,and then analyze lhe correlation with ADC values of the tumor tissue. Results The mean signal intensity and ADC values of tumor tissue of endumeutrial caueer were ( 172.37 ± 14. 12 ) and ( 0.94 ±0. 17 ) mm^2/s. The ADC values of Moderately differentiated endometrial cancer was significantly lower than high differentiated endometrial cancer ( P 〈 0.05 ) , Dut there was no significantly difference about signal intensity ;the ADC values were currected negatively with the expressions of PCNA of eudumetrial cancer. Conclusion Quantitative measure the ADC values of the tumor tissue have a important (clinical value for different histologic. grade in endometrial cancer,and can have are a tain exte.nt to reflect the expressions of PCNA of tumor tissue.
