摘要
目的观察血清 TGF-β1与肝组织病理和纤维定量之间的确切关系,探讨其临床应用价值.方法采用酶联免疫吸附实验(ELISA)测定76例慢性乙肝(CHB)、肝硬变(LC)患者血清 TGF-β1.30例作肝活检,进行肝组织病理学诊断,应用多媒体彩色病理图文分析系统对肝内胶原纤维、网状纤维进行定量分析.结果①CHB 轻、中、重度、LC 患者血清 TGF-β1水平(144.3μg/mL±34.1 μg/mL,349.5μg/mL±107.7μg/mL,508.3 μg/mL±133.3μg/mL,579.4 μg/mL±225.1μg/mL)明显高于对照组(95.3μg/mL±34.1 μg/mL;P<0.01).按CHB 轻、中、重度、LC 依次升高(P<0.01/0.05).②肝组织内胶原纤维、网状纤维含量随 CHB 轻、中、重度次序递增(胶原纤维的单位目标面积在肝实质为(μm^2/个):344.0±9.6,43.7±11.9,64.2±10.7;汇管区为:66.5±17.3,75.5±23.4,91.0±22.8网状纤维的单位目标面积在肝实质为(μm^2/个):11.4±5.0,17.4±4.1,26.3±9.7汇管区为:18.0±5.7,24.6±6.1,79.3±27.0 P<0.01/0.05),与血清 TGF-β1的变化一致.结论血清 TGF-β1与肝组织病理和肝组织纤维定量呈一致性改变.
AIM To observe the relation between serum TGF-β1 of patients with chronic hepatitis B (CHB) and hepatic tissue pathology as well as hepatic fibrosis quantity. METHODS Serum TGF-β1 was detected by ELISA.Liver biopsy was performed in 30 of 76 patients with CHB. Diagnosis of pathohistology and quantitative analysis of collagenous and reticular fibers were made. RESULTS ①Serum TGF-β1 levels in CHB and LC (144.3mg/L±34.1mg/L,349.5mg/L±107.7mg/L, 508.3mg/L±133.3mg/L,579.4mg/L±225.1mg/L) were higher than in NC (95.3mg/L±34.1mg/L; P<0.01) and increased successively in mild,moderate and severe CriB and LC (P<0.01/0.05).②The quantity of hepatic collagenous and reticular fibers went up gradually in mild,moderate and severe CHB (μm^2/个, 344.0±9.6,43.7±11.9,64.2±10.7,66.5±17.3,75.5± 23.4,91.0±22.8,11.4±5.0,17.4±4.1,26.3±9.7, 18.0±5.7,24.6±6.1,79.3±27.0 P<0.01/0.05). Meanwhile,serum TGF-β1 rose to a same degree. CONCLUSION Changes among serum TGF-β1 of patients with CHB and hepatic tissue pathology as well as hepatic fibrosis quantity was consistent.
