摘要
目的对异位妊娠未破裂组和卵巢黄体囊肿组进行超声对比研究,以提高未破裂型异位妊娠的早期诊断。方法使用三维能量多普勒超声观察180例正常早孕者(对照组)、155例正常早孕且伴有黄体囊肿者(黄体囊肿组)和84例未破裂异位妊娠患者(异位妊娠未破裂组)的超声图像,采集并对比血流动力学参数,评价其在诊断中的意义。结果对照组、黄体囊肿组和异位妊娠未破裂组间周边环状血流的收缩期峰值流速的均值差异无统计学意义;异位妊娠未破裂组与对照组组间阻力指数与搏动指数的均值差异无统计学意义;异位妊娠未破裂组与黄体囊肿组阻力指数与搏动指数的均值差异有统计学意义(P<0.05);异位妊娠未破裂组与卵巢黄体囊肿组、异位妊娠未破裂组与对照组组间血管形成指数、血流指数、血管形成-血流指数差异均有统计学意义(P<0.05)。结论三维能量多普勒超声能为异位妊娠未破裂组与黄体囊肿的鉴别诊断提供有价值的信息,可进一步提高异位妊娠的诊断准确性,对早期诊断异位妊娠有重要意义。
Objective To enhance the early diagnosis of unruptured ectopic pregnancy by comparing the unruptured ectopic pregnancy group and the luteum cyst group.Methods Threedimensional power Doppler ultrasound was used on 180 patients with cases of normal pregnancy as control group,155 normal pregnancy and those associated with luteum cyst as luteum cyst group,84 cases with ectopic pregnancy mass as unruptured ectopic pregnancy group.The twodimensional image and the Doppler hemodynamic parameters of the three groups were compared and the corresponding blood parameters were measured to evaluate the significance of the three groups in diagnosis.Results The three groups＇ peak systolic velocity had no statistically significant differences;there were no statistically significant differences found between the unruptured ectopic pregnancy group and the control group＇s resistance index,pulsatility index;there were statistically significant differences in the unruptured ectopic pregnancy group and the luteum cyst group＇s resistance index,pulsatility index（P 0.05）;there were statistically significant differences in the unruptured ectopic pregnancy group and the luteum cyst group,and also in the unruptured ectopic pregnancy group and the control group＇s vascularization,flow index,vascularization-flow index（P 0.05）.Conclusion Three-dimensional power Doppler ultrasound can provide valuable information that can further improve the diagnosis of ectopic pregnancy.
