糖尿病足发生发展的危险因素 被引量:32

摘要 目的探讨2型糖尿病患者糖尿病足(DF)发生发展的危险因素。方法收集2006~2010年收治于武汉大学人民医院内分泌科,临床资料相对完整的186例单纯糖尿病患者及136例合并DF患者,按住院期间是否行截肢手术,将DF患者分成截肢组和非截肢组。回顾性分析患者的一般资料、既往病史、合并慢性并发症、实验室检查指标及DF的局部评估,并进行多因素非条件Logistic回归分析。结果与单纯糖尿病患者比较,DF患者年龄、踝肱指数(ABI)、糖尿病病程、高血压病史、收缩压、合并糖尿病肾病(DN)、周围神经病变、下肢血管闭塞改变、空腹血糖、血脂紊乱、血肌酐(Scr)、尿素氮(BUN)、血浆白蛋白(Ab)、血红蛋白(Hb)、超敏C反应蛋白(hs-CRP)、血白细胞计数、血凝状态回顾指标差异显著;截肢组与非截肢组患者ABI、周围神经病变、下肢血管病变、Ab、hs-CRP、白细胞计数、纤维蛋白原(FIB)指标有统计学意义差异;DF局部评估示截肢率与Wanger分级呈正相关,截肢组有更高的G-杆菌感染率,且多见需氧菌及兼性厌氧菌混合感染;多因素非条件Logistic回归分析示低ABI、空腹血糖升高、血脂紊乱、hs-CRP及FIB升高、低Hb是DF发生的危险因素;ABI减小、低Ab、高hs-CRP、高白细胞计数、DF严重程度是DF患者发展为截肢的危险因素。结论 DF临床防治工作必须建立多学科的协作机制,宣教DF知识,加强个人防护知识教育,积极控制血糖、血脂、血压及慢性并发症,纠正贫血、低蛋白血症等进行早期干预,对已发生DF的患者,在DF清创换药控制感染的同时,还要改善下肢血供和进行营养支持,以减少DF截肢的发生。
