不停跳冠脉搭桥患者围术期血浆脑钠肽水平的变化及其临床意义 被引量:3

Perioperative changes and clinical significance of serum brain natriuretic peptide in patients undergoing off-pump coronary artery bypass grafting

摘要 目的探讨不停跳冠脉搭桥术患者血浆脑钠肽(BNP)水平围术期的变化及临床意义。方法不停跳冠脉搭桥术患者45例,检测术前及术后第1天、第3天、第7天血浆BNP水平。45例患者分为术前心功能Ⅰ级+Ⅱ级11例(A组)、Ⅲ级20例(B组)、Ⅳ级14例(C组);术后出现并发症9例(D组),无并发症36例(E组)。20例健康体检者为F组。结果 45例患者术后第1天BNP开始升高,第3天达峰并开始下降,第7天仍高于术前(P<0.05);A、B、C三组BNP呈上升趋势,且显著高于F组(P<0.05);D组BNP明显高于E组(P<0.05)。结论不停跳冠脉搭桥术患者围术期血浆BNP水平可反映术者心脏功能,对术前评估、判断预后有重要临床意义。 Objective To investigate the dynamic changes and clinical significance of serum brain natriuretic peptide(BNP) in patients undergoing off-pump coronary artery bypass grafting(OPCABG).Methods On the basis of cardiac function(NYHA),forty-five patients were divided into three groups of A(grade Ⅰ+Ⅱ,11 cases),B(grade Ⅲ,20 cases),C(grade Ⅳ,14 cases),who were assigned into groups of D(9 cases with postoperative complications) and E(36 cases without postoperative complications).Another 20 healthy people were taken as the controls(group F).Serum BNP was measured before operation,on the 1st,3rd and 7th day after OPCABG.Results The serum BNP level elevated markedly on the 1st day after operation and reached peak value on the 3rd day,which then decreased but was still higher than that before on the 7th day(P0.05).BNP levels after operation in groups of A,B and C were elevated gradually,which were higher than those in group F(P0.05).The preoperative BNP level was significantly higher in group D than that in group E(P0.05).Conclusion The perioperative serum BNP level could reflect the cardiac function and is valuable in evaluating and predicting the prognosis of the patients underwent OPCABG.
