Quantification of Liver Fat Content by ~1H-MR Spectroscopy Imaging at 3.0 T for Therapeutic Evaluation of Fatty Liver——Preliminary Study

摘要 目的探讨在3.0 T MRI中运用氢质子波谱成像(1 H-MRS)技术评估脂肪肝治疗效果的可行性。方法收集临床确诊并治疗的脂肪肝患者26例,分别于治疗前、治疗后3和6个月各行1次1 H-MRS检查,测得1 H-MRS的水峰峰值和脂肪峰峰值、水峰峰下面积和脂肪峰峰下面积,计算肝细胞相对脂肪含量1(RLC1)及相对脂肪含量2(RLC2);同期测量患者的甘油三酯、γ-谷氨酰转肽酶、腹围及体重指数,将其拟合成临床脂肪肝指数(fatty liver index,FLI)。以FLI为参照标准,对不同时间点1 H-MRS所测得肝脏脂肪含量进行统计学分析。结果采用配伍设计的方差分析,治疗前、治疗后3和6个月的RLC1、RLC2值比较差异有统计学意义(P<0.05);用SNK法做两两检验,治疗前RLC1、RLC2值均分别明显高于治疗后3个月或治疗后6个月的RLC1、RLC2值(P<0.05)。RLC1、RLC2与FLI进行秩相关性分析,均呈正相关性(r=0.476,P<0.001;r=0.475,P<0.001)。可靠性分析显示,治疗前的组内相关系数均≥0.75,可重复性好。结论 1 H-MRS定量分析评估脂肪肝治疗效果具有可行性,并且是一种无创的定量监测方法。 Objective To investigate the feasibility of proton magnetic resonance spectroscopy(1H-MRS) imaging,by which to quantitatively analyze liver fat content for therapeutic evaluation of fatty liver at 3.0 T MRI. Methods Twenty-six patients who diagnosed with fatty liver were examined with proton MRS at Siemens Trio Tim 3.0 T MRI before treatment and 3,6 months after treatment,respectively.The water peak,fat peak,water peak area,and fat peak area were detected,and the relative lipid content 1(RLC1) and relative lipid content 2(RLC2)were calculated.Fatty liver index(FLI) was referred to the standard which was calculated from triglycerides(TG),gamma-glutamyl-transferase(GGT),waist circumference,and body mass index. Results There were significantly different differences of RLC1 and RLC2 among before treatment and 3,6 months after treatment(P〈0.05).Compared with before treatment,the RLC1 and RLC2 values significantly decreased on month 3 or 6 after treatment(P〈0.05).There were positive correlation between RLC1 or RLC2 and FLI(r=0.476,P〈0.00;r=0.475,P〈0.001).The intraclass correlation coefficient was more than 0.75 before treatment,the repeatability was better. Conclusions Proton MRS can quantitatively measure liver fat content.It can be reliably used for dynamic monitoring the therapeutic effects for fatty liver.Proton MRS is accurate,and has a good clinical application in dynamically monitoring the progression of fatty liver and evaluating the therapeutic effects of various treatments
