熔点曲线法研究HBV基因B型BCP区变异与临床表现的关系 被引量:2

摘要 目的研究乙型肝炎病毒(HBV)基因B型基本核心启动子(BCP)的变异及其临床意义。方法收集HBV携带者、慢性乙型肝炎、慢性重型肝炎等3种临床类型的患者血清226份,采用巢式多聚酶链式反应(PCR)扩增HBV DNA,选取上述3种类型各50例B基因型患者,采用熔点曲线方法进行BCP区的变异检测。结果 226例患者中B基因型210例(92.9%),C基因型16例(7.1%);150例不同病情的B基因型患者中,慢性重型肝炎患者的熔点曲线平均波峰数量多于慢性HBV携带者及慢性乙型肝炎患者,差异有统计学意义(P<0.05),而HBV携带者与慢性乙型肝炎比较,差异无统计学意义(P>0.05)。结论 B基因型患者BCP区变异与临床表现存在一定的关系,病情重者准种数量较多。 [ Objective] To study the clinical significance of basic core promoter (BCP) mutants of genotype B with hepatitis B virus (HBV). [Methods] Detected HBVDNA in serum samples from 226 patients with HBV infection including chronic asymptomatie carriers, chronic hepatitis B and chronic severe hepatitis by nested chain reaction (nested-PCR), then melting curve analysis was applied to find the BCP quasispeeies of genotype B existing in such three groups containing 50 cases separately. [Results] 210 samples of genotypes B (92.9%) and 16 samples of genotype C (7.1%) were found. According to the mehing curve analysis, the number of wave crests in the chronic severe'patients was significantly more than that of the chronic asymp- tomatic carriers and chronic hepatitis B patients (P〈0.05), while there was no significant difference between the chronic asymptomatic carriers and chronic hepatitis B patients (P〉0.05). [Conclusions] The complication of gene variant strain or quasispecies of BCP may contributed to the clinical exacerbation with HBV infection, in the patients infected with HBV of genotype B, those who have more quasispecies of BCP would have more severe clinical manifestations.
