摘要
目的:探讨椎基底动脉CT血管造影对椎基底动脉系统短暂性脑缺血发作病因的诊断价值。方法:临床诊断为椎基底动脉系统短暂性脑缺血发作患者30例编入TIA组,选取同期40例体检者编入对照组,均行常规CT和CT血管造影。评估椎基底动脉的病变,包括椎基底动脉狭窄、走行迂曲、变异。结果:30例TIA组25例有椎基底动脉异常表现:椎基底动脉管腔狭窄27支;狭窄病因:动脉粥样硬化斑块阻塞管腔14支,椎动脉迂曲致局部狭窄6支,骨质增生压迫椎动脉狭窄2支,先天发育细小5支;狭窄部位:V1段5支,V2段3支,V3段3支,V4段13支,基底动脉狭窄3支;其中轻度狭窄8支,中度狭窄11支,重度狭窄7支,闭塞1支;椎动脉明显迂曲6支;基底动脉起源异常1支;椎动脉一侧发育细小3支;椎动脉窗1支。40例对照组,8例8支椎基底动脉狭窄,其中轻度狭窄6支,中度狭窄2支,同时伴椎动脉迂曲1支。两组暴露率有统计学差异(2=27.59,P<0.001)。椎基底动脉异常是发生椎动脉系统短暂性脑缺血发作的危险因素(OR=20)。结论:CTA是一种很好的显示椎基底动脉解剖关系及其病变的检查方法,对椎基底动脉系统短暂性脑缺血发作病因有很好的显示价值。
Objective:To explore the etiological diagnostic value of vertebrobasilar CTA in vertebrobasilar transient ischemic attack(TIA). Methods:A total of 30 cases with vertebrobasilar TIA and a total of 40 healthy persons were performed with conventional CT and 16-SCTA.Vertebrobasilar lesions including vertebrobasilar stenosis,vertebrobasilar abnormal course,and vertebrobasilar variation were evaluated. Results: Of these 30 cases with TIA,25 cases with vertebrobasilar lesion were found by CTA.The vertebrobasilar stenoses in 5 cases were found by CTA,including atherosclerosis plaque obstructed vessels in 14 arteries,vertebrobasilar abnormal course compressed vessels in 6 arteries,bone hyperplasia compressed vessels in 2 arteries,hypoplasia stenosis in 5 arteries.There were 27 narrow arteries in which 5 were in V1 segment,3 were in V2 segment,3 were in V3 segment,13 were in V4 segment,and 3 were in basilar artery segment.In all 27 narrow arteries,8 arteries were mild stenosis,11 arteries were moderate stenosis,1 arteries was occlusion.In the control groud,8 cases had vertebrobasilar stenosis in which 6 arteries were mild stenosis,2 arteries were moderate stenosis,and 1 artery was vertebrobasilar abnormal course.Exposure rates in two groups were obviously different(2=27.59,P0.001).Vertebrobasilar lesion was a risk factor of vertebrobasilar TIA(OR=20).Conclusion:CTA is an accurate way in diagnosis of vertebrobasilar adjacent anatomical structure and its lesion.CTA can clearly show the etiology of vertebrobasilar TIA.
