摘要
目的:探讨胆囊切除致肝外胆管损伤的处理方法。方法:对10例医源性胆管损伤的临床资料进行回顾性分析。结果:10例中8例为开腹胆囊切除术损伤,2例为腹腔镜胆囊切除术损伤。术中发现6例,术后早期确诊3例,后期确诊1例。手术方式:胆管端端吻合1例,胆总管十二指肠吻合1例,胆管壁修补1例,胆管空肳Roux-en-Y吻合7例。结论:正确掌握胆囊切除致肝外胆管损伤的处理原则及方法。胆管空肠Roux-en-Y是常用而有效的方法。
Objective：To summarize the treatment experiences of extrahepatic bile duct injury caused by cholecystectomy.Methods： The clinical data of 10 cases of iatrogenic bile duct injury were analyzed retrospectively.Results：Among 10 cases,8 cases were damaged by open cholecystectomy,and the other cases by laparoscopic cholecystectomy.6 cases were found during operation.3 cases were diagnosed during early postoperation.1 case was diagnosed during late postoperation.The operation modes were as follows： end to end choledochocholedochostomy in 2 cases,common bile duct and duodenal anastomosis in 1 case,reparing of duct in 1 case and Roux-en-Y cholangiojejunostomy in 7 cases.Conclusion：The principles and methods to treat extrahepatic bile duct injury caused by cholecystectomy should be correctly mastered.Roux-en-Y cholangiojejunostomy is a common and effective method.
