双水平无创正压通气治疗COPD并发Ⅱ型呼吸衰竭的疗效观察 被引量:5

摘要 目的观察双水平无创正压通气法(BiPAP)在治疗COPD急性加重期并发Ⅱ型呼吸衰竭患者的应用效果。方法22例COPD并发Ⅱ型呼吸衰竭的患者分为治疗组12例和对照组10例,治疗组在常规治疗的基础上给予双水平无创正压通气治疗,对照组仅给予常规治疗,观察治疗前后24h的动脉血气分析、心率、呼吸频率的变化。结果治疗前后血气分析表明,治疗组pH明显改善,PaO2明显升高,PaCO2、心率、呼吸频率明显降低(P<0.05)。对照组相关指标无明显改善。结论 BiPAP治疗COPD急性加重期并发Ⅱ型呼吸衰竭患者疗效好,能显著降低PaCO2、呼吸频率,提高PaO2,可以缩短病程。 Objective To observe the clinical effect of noninvasive positive pressure ventilation ( NPPV ) in the treament of chronic obstructive pulmonary disease at acute exacerbation phase combined with type Ⅱ respiratory failure. Methods Twenty-two patients of COPD complicated with type Ⅱ respiratory failure were assignel to treatment group with 12 patients or control gyoup with 10 patients. In treatment group were treated with BiPAP ventilation in addition to above routine treatment. Control group received conventional management consisted of antibiotics, apophlegmatic, anticonvulisive, respiratory, stimulant, and long-term low-dose oxygen therapy. observe two groups were breathing; heart rate blood gas analysis, the leagth of hospitalilation time for index. Results The indexes of arterial blood gas assays were improved: In the treatment group , pH significant improvemented, PaO2 increased ; heart rates. respiratory rates and PaCO2 decreased (P 〈 0.05 ). There was not significant improvemented in the control gyoup. Conclusion NPPV intervention can effectively decrease PaCO2 and RR , increase PaO2 and shorten the course of hospitalization.
