摘要
目的结合循证医学(evidence-based medicine,EBM)相关理论,探讨无创正压通气(noninvasive positive pressure ventilation,NIPPV)在治疗慢性阻塞性肺疾病(chronic obstructive pulmo-nary disease,COPD)合并呼吸衰竭的应用价值与临床疗效。方法以EBM为理论指导,收集我院2007年1月至2011年2月呼吸内科共72例COPD合并呼吸衰竭患者,随机分为对照组(32例)和治疗组(40例),对照组接受常规药物治疗,治疗组在其基础上应用面罩以积极的呼吸道压力(bi-level posi-tive airways pressure,BiPAP)模式行NIPPV治疗。结果治疗组示NIPPV治疗后2～3h及24h血气分析,PaCO2、PaO2、pH值等及气管插管率、住院时间均较入院时改善,较对照组差异有统计学意义(P<0.05)。结论在BIPAP模式行NIPPV治疗COPD合并呼吸衰竭,可迅速改善该类患者血气情况,减少气管插管率与住院时间等,是临床治疗本病的有力举措。
Objective To explore the application value and clinical efficacy of noninvasive positive pressure ventilation on the treatment of chronic obstructive pulmonary disease combined with respiratory failure, based on the correlative theory of evidence-based medicine. Methods Under the guidance of the evidence-based medicine theory,72 patients with chronic obstructive pulmonary disease combined respiratory failure in hospital from Jan. 2007 to Feb. 2011 were divided into con- trol group （32 cases） and treatment group （40 cases） randomly, then treated with routine medicine and noninvasive positive pressure ventilation by bi-level positive airways pressure model respective- ly. Results Compared with hospitalized, the PaCO2, PaOz, pH value, endotracheal intubarion rate and hospital days of the treatment group were improved after treatment for 2-3 h and 24 h, which had statistical significance compared with the control group. Conclusion Noninvasive posi- tive pressure ventilation by bi-level positive airways pressure model can rapidly improve the bloodgas situation, reduce endotracheal intubarion rate and hospital days of the patients, etc, which was the effective method for the treatment of the disease.
