摘要
目的探讨不典型肝脓肿64层螺旋CT三期增强扫描的特征性表现。方法回顾性分析29例经穿刺活检或手术病理证实的不典型肝脓肿的64层螺旋CT三期增强表现。结果 29例不典型肝脓肿的64层螺旋CT三期增强扫描可见:(1)动脉期:27例出现病灶周围片状或楔形的一过性强化,病灶内无分支血管影。(2)门静脉期:27例病灶花环状或分隔状强化,表现为乳突气房征;18例出现晕征。(3)延迟期:26例显示延时强化征象;28例病灶范围比平扫时缩小。结论不典型肝脓肿的64层螺旋CT三期动态增强扫描具有特征性表现,有助于提高不典型肝脓肿的诊断及鉴别诊断。
Objective To evaluate the 64-slices spiral CT typical findings of atypical hepatic abscesses of triple-phase enhanced scanning.Methods The 64-slices spiral CT findings of atypical hepatic abscesses,diagnosed by pathology in 29 cases were retrospectively analyzed.Results In the arterial phase,27 cases showed patchy or wedge-shaped enhancement around the abscesses provisionally,with no artery into the abscesses.In the portal vein phase,27 cases showed circular or disjunctive enhancement,performed as mastoid air cell sign,and 18 cases showed halo sign.In the delayed phase,26 cases showed delayed enhancement sign,and 28 cases showed smaller sign than the plain scan.Conclusion The 64-slice spiral CT findings of the atypical hepatic abscesses of triple-phase enhanced scanning have typical features,which are helpful for the diagnosis and differential diagnosis of the atypical hepatic abscesses.
