多排螺旋CT冠脉成像在冠心病诊断中的临床价值 被引量:5

摘要 目的:探讨冠心病诊断中多排螺旋CT冠脉成像的临床应用价值。方法:59例疑诊冠状动脉狭窄患者行多排螺旋CT检查,了解冠状动脉病变情况,同时行冠状动脉造影进行对比。结果:59例271支血管同时经多排螺旋CT和冠状动脉造影成像。多排螺旋CT发现96支冠状动脉狭窄,其中左前降46支,回旋支15支,左主干7支,右冠状动脉25支,桥支3支。冠状动脉造影检查发现91支冠状动脉狭窄,左前降支42支,回旋支16支,左主干6支,右冠状动脉23支,桥支4支。多排螺旋CT与冠状动脉造影结果相符的病变血管84支,敏感性为92%,特异性为93%,阳性预测值为88%,阴性预测值为96%。结论:在图像质量能够满足影像学评价的基础上,多排螺旋CT可作为冠状动脉狭窄的一种可靠的无创筛选检查方法,对冠心病的诊断有着重要的临床价值。 Objective:To evaluate the clinical value of multislice spiral CT(MSCT) imaging in the diagnosis of coronary artery disease.Methods:Fifty-nine patients with suspected coronary artery diseases were underwent both MSCT and coronary angiography(CAG),then the outcomes were compared.Results:Two hundred and seventy one coronary artery segments of 59 patients were evaluated by both MSCT and CAG.Ninety-six stenosis coronary arteries were found by MSCT,including 46 left anterior descending coronary arteries(LAD),15 left circumflex coronary arteries(LCX),7 left main coronary arteries(LM),25 right coronary arteries(RCA) and 3 bridge coronary arteries.Ninety-one stenosis coronary arteries were found by CAG,including 42 LAD,16 LCX,6 LM,23 RCA and 4 bridge coronary arteries.There were 84 stenosis coronary artery segments,which were found by both MSCT and CAG.The global sensitivity,specificity,positive predictive value,negative predictive value of MSCT were 92%,93%,88% and 96%,respectively.Conclusion:Based on the satisfied image quality, MSCT can be a reliable noninvasive screening examination for coronary artery stenosis,it plays a great potential role in diagnosis of coronary artery disease.
