摘要
目的探讨慢性酒精中毒患者血清脂联素(APN)、叶酸(FA)、维生素B12水平的变化及其与甘油三酯、胆固醇、HDL-C、LDL-C、空腹血糖、谷草转氨酶、谷丙转氨酶等指标的相关性,进一步探讨慢性酒精中毒与脑血管疾病及其高危因素的关系,为临床预防和治疗慢性酒精中毒患者、观察其预后均有重要的临床价值。方法慢性酒精中毒患者54例,年龄37～68岁,平均48.42±6.39岁。饮酒年限为5～42年,纳入标准符合《中国精神障碍分类与诊断标准-CCMD3》;正常对照组43例,年龄24～59岁,平均32.60±7.00岁。采用ELISA法测定血清中脂联素水平;用化学发光法测定叶酸和B12水平,同时测定空腹血糖、甘油三酯、胆固醇、HDL-C、LDL-C、谷草转氨酶、谷丙转氨酶等相关指标的水平,研究它们与血清脂联素、叶酸、B12的关系。结果 (1)脂联素(mg/L):对照组平均水平为2.05±1.10(mg/L);酒精组脂联素平均水平为1.5±0.57(mg/L)。Pearson相关分析表明:脂联素与空腹血糖呈负相关;与叶酸、B12、甘油三酯、胆固醇、HDL-C、LDL-C无明显相关性。(2)叶酸(ng/ml):对照组为7.33±2.4;酒精组为3.63±1.97。Pearson相关分析表明:叶酸与B12、甘油三酯、HDL-C呈负相关;与脂联素、胆固醇、空腹血糖、LDL-C无明显相关性。(3)B12(pg/ml):对照组403.4±116.17;酒精组497.57±289.57。Pearson相关分析表明:与叶酸呈负相关;与HDL-C呈正相关;与脂联素、胆固醇、LDL-C、空腹血糖无明显相关性。结论 (1)长期饮酒可降低血清APN水平,且血清APN水平与血糖呈负相关。随着饮酒年限的增加,未发现血清APN随之降低的规律性。(2)长期饮酒可引起血清FA水平降低,并且随着饮酒年限的逐年增加,叶酸FA降低越明显。
Objective To explore the clinical significance of changes of serum adiponectin（APN） levels,serum folic acid（FA） levels and serum VitB12（VB12） levels in patients with chronic alcoholism,and to observe the relationship of triglyceride（TG）,total chrolesterol（TC）,fasting blood glucose（FBG）,HDL-C and LDL-C,and try to further explore the relationships of changes between the cerebrovascular diseases and their high risk factors in patients with chronic alcoholism.Methods 54 cases of patients were selected,age between 37 to 68 years old,the average age of（48.42±6.39） years,drinking for 5~42 years.They were diagonosed according to the Chinese classification and the diagnostic criteria for mental disorders（CCDM3）.43 cases of control group,age between 24 to 59 years old,the average age of（32.60±7.00） years.Serum adiponectin（with ELISA）,serum folic acid and vitB12 levels（with RIA） levels were determined,and fasting blood glucose（FBG）,TG,TC,HDL-C,LDL-C were measured in the morning.Results The average adiponectin levels were 2.05±1.10（mg/L）for the control group and 1.5±0.57（mg/L） for the patient group.Pearson correlation analysis indicated that adiponectin was negatively correlated with fasting blood glucose（FBG）.There was no significant correlation with FA,VB12,TG,TC,HDL-C and LDL-C.The average FA levels were 7.33±2.40（ng/ml）for the control group and 3.66±1.97（ng/ml） for the patient group.Folic acid was negatively correlated with VB12,TG and HDL-C.There was no significant correlation with adiponectin,TC,FBG and LDL-C.（3）VB12（pg/ml）：The average VitB12 levels were 403.4±116.17（pg/ml） for the control group and 497.57±289.57（pg/L） for the patient group.VitB12 was negatively correlated with folic acid（FA）.VitB12 was positively correlated with HDL-C.There was no significant correlation with APN,TC,FBG,LDL-C.Conclusion Serum adiponectin in chronic alcoholism patients was significantly lower than those in control group.Serum folic acid in chronic alcoholism patients was significantly lower than those in control group.
