摘要
目的探讨营养支持对机械通气患者免疫功能及临床疗效的影响。方法将56例机械通气患者随机分为两组,观察组28例,对照组28例,在治疗原发病和生命支持的同时,观察组给予ω-3脂肪酸和谷氨酰胺+常规肠内营养,对照组单纯常规肠内营养。对比两组患者治疗前后营养代谢、免疫指标的变化、机械通气时间及28 d病死率。结果治疗后两组TNF-αI、L-6水平较治疗前均有下降(P<0.05),治疗7 d时观察组水平较对照组下降更明显,差异有统计学意义(P<0.05);观察组前白蛋白较治疗前明显上升(P<0.05),机械通气时间缩短(P<0.05);但两组28 d病死率差异无统计学意义(P>0.05)。结论对机械通气的患者应积极给予营养支持,常规肠内营养加ω-3脂肪酸和谷氨酰胺有利于快速改善患者的营养状态,缩短机械通气时间。
Objective To evaluate the effect of nutritional support treatment between those treatments in treating patients with mechanical ventilation. Methods 56 patients using mechanical ventilation were randomly divided into the control group, the glutamine and to-3 polyunsaturated fatty acids ( ω-3 PUFA) treatment group, the change of every groups patients'indexes, such as ALB, prealbumin,TNF-α, IL-6,ventilation duration were observed before and 7 days later after treatment,incidence of 28-day mortality between the treatment group and the contrast group were compared. Results The levels of TNF-α,IL-6 in the two groups were lower compared with the baseline values before the treatment(P 〈0.05), TNF-α, IL-6 in treatment group were lower than those in control group at according time ( P 〈 0. 05 ) ; the indexes of prealbumin in treatment group were obviously higher than those in the contrast group (P 〈 0.05 ). The days of mechanical ventilation were significantly lower in study group ( P 〈 0. 05 ). While there were no significant difference in 28-day mortality between treatment group and contrast group(P 〉0.05). Conclusion Nutritional supporting therapy is of great benefit to the patients with mechanical ventilation, glutamine and ω-3 PUFA can improve the immune function and nutritional status in critical patients significantly.
