摘要
目的:分析妊娠期糖尿病(GDM)并妊娠高血压疾病孕妇孕中期血清尿素氮、肌酐和尿酸水平的变化,便于发现早期异常指标,以利早期防范。方法:观察比较112例GDM(A组)、38例GDM并妊娠高血压疾病(B组)和32例75g葡萄糖耐量试验(OGTT)正常的孕妇(NGT组)孕中期血清尿素氮、肌酐和尿酸水平的变化。结果:与NGT及A组相比,B组FPG、2 h PG、HbA1C、血清尿素氮、肌酐和尿酸增高,差异有统计学意义(P<0.01);A组与NGT组相比,FPG、2hPG、HbA1C增高,差异有统计学意义(P<0.01),血清尿素氮、肌酐和尿酸差异无统计学意义(P>0.05)。结论:GDM孕期血糖控制不良时,如发现血清尿素氮、肌酐和尿酸浓度值在正常范围上限或明显高于正常孕妇,就应高度警惕,及早做好防范措施,避免将来妊娠高血压疾病的发生。
Objective：To analyze the changes of serum urea nitrogen,creatinine and uric acid levels in pregnant women with gestational diabetes mellitus（GDM） combined with hypertensive disorder complicating pregnancy（HDCP）,in order to find abnormal indexes early and take preventive measures early.Methods：The changes of serum urea nitrogen,creatinine and uric acid levels during the second trimester of pregnancy in 112 pregnant women with GDM（A group）,38 pregnant women with GDM combined with HDCP（B group） and 32 pregnant women with normal 75 g oral glucose tolerance test（OGTT）（NGT group） were observed and compared.Results：Compared with NGT group and A group,FPG,2-hour PG,HbA1C,serum urea nitrogen,creatinine and uric acid in B group increased significantly（P0.01）.Compared with NGT group,FPG,2-hour PG and HbA1C in A group increased significantly（P0.01）,there was no significant difference in serum urea nitrogen,creatinine and uric acid levels between A group and NGT group（P0.05）.Conclusion：When GDM can not be controlled well during pregnancy,and serum urea nitrogen,creatinine and uric acid levels are higher than those of normal pregnant women,preventive measures should be taken to avoid the occurrence of HDCP.
