微创颅内血肿清除术治疗高血压脑出血的临床分析 被引量:12

摘要 目的探讨微创颅内血肿清除术治疗高血压脑出血的临床效果。方法根据CT导向定位或CT片定位,采用YL-1型一次性颅内血肿粉碎穿刺针,配合生物溶酶技术进行冲洗、液化、引流清除血肿。结果本组32例,定位准确率100%,存活25例,死亡7例;3例并发呼吸衰竭死亡;2例死于术后7 d多器官衰竭,3例术后3 d放弃治疗回当地医院死亡2例、存活1例。1例再出血改开颅手术后治愈。血肿清除时间1-6 d。结论微创颅内血肿清除术治疗高血压脑出血是一种操作简单、创伤小、手术时间短、疗效确切、费用低,特别适用于基层医院推广使用。 Objective To summarize our experience in minimal invasive technique for hypertensive intracranial hematomas(HICH).Methods There were 32 patients with HICH who were treated in our hospital between May 2007 and August 2010.The HICH was excavated and drained using YL-1 needle guided by CT images.Results HICH was precisely located in all cases.The patients were alive in 25 cases and dead in 7.Three patients died of respiratory failure,and two patients died of multiple organ system failure;3 patients discharged 3 d postoperatively and 2 patients of them died.One patient was reoperated due to rebleeding.Conclusion Minimal invasive technique using YL-1 needle is effective,simple,time-saving methods for HICH,and can be widely used in primary hospital.
