应用一种新型无创技术筛查糖代谢异常的临床研究 被引量:9

摘要 目的评估一种以电化学为原理、利用离子渗透技术的无创设备(EZSCAN)筛查糖尿病(DM)、糖耐量受损(IGT)和代谢综合征(MS)的效能。方法既往无明确血糖异常者166例,常规体检,分别应用经典方法空腹血糖(FPG)、糖化血红蛋白(HbA1c)、糖耐量试验(OGTF)和EZSCAN检测个体血糖状态。运用方差分析(GLM)、SNK分析,并运用logistic回归分析、ROC分析评价诊断价值。结果经典方法和EZSCAN检测出DM4例;IGT26例;FPG正常(NGT)合并MS25例;FPG正常且无MS111例。经典方法检测FPG处于临界水平(7.0mmol/L)时,检测DM的敏感性低(0%)。而EZSCAN检测的敏感性,对DM为50%,对IGT为77%,对MS为64%。结论在本研究组中,用FPG检测DM敏感性较低。而使用EZSCAN技术检测,不仅对DM和IGT都具有良好的敏感性,而且对于识别NGT合并MS也具有良好的敏感性。EZSCAN新技术可能是IGT、DM及MS早期筛查的有效方法。 Objective To assess the efficiency of an iontophoresis-based screening method (EZSCAN) in the detection of impaired glucose tolerance (IGT), diabetes mellitus (DM) and metabolic syndrome ( MS). Methods A total of 166 subjects without medical history of dysglyeaemia underwent fasting plasma glucose (FPG) and HbAlc measurement and oral glucose tolerance test (OGTF) by using traditional or EZSCAN method. Variance analysis ( GLM ), SNK analysis, logistic regression analysis, and ROC analysis were used to evaluate the diagnostic value of those screening techniques. Results DM, IGT, nornlal glucose tolerance (NGT) + MS, and NGT were found in 4, 26, 25 or 111 participants, respectively. For traditional test,FPG of 7. 0 mmoL/L showed a lower sensitivity to detect DM (0%). The sensitivity of EZSCAN to detect DM ,IGT or MS was 50% ,77% and 64% ,respectively. Conclusions FPG may have lower sensitivity to detect DM, although EZSCAN could show higher sensitivity to detect IGT, DM, and MS.
