摘要
目的:通过模拟分娩产伤和雌激素水平下降联合阴部神经切断两种方法,建立压力性尿失禁(SUI)的动物模型,并对所建模型进行鉴定和比较,为SUI的研究提供可靠易得的实验对象。方法:40只健康雌性Wistar大鼠随机分为4组,Ⅰ组为对照组;Ⅱ组为阴道扩张+卵巢切除组;Ⅲ组为阴部神经切断组;Ⅳ组为阴道扩张+卵巢切除+阴部神经切断组,每组10只。术后1、4、8周对4组大鼠行尿动力学检测,1周时做喷嚏负荷试验以证实SUI的存在。8周后取大鼠全层尿道及盆底肌群组织做组织学鉴定。结果:①喷嚏负荷试验,Ⅱ、Ⅲ、Ⅳ组分别有6、9、10只阳性,而Ⅰ组无阳性。②漏尿点压(LPP),Ⅲ组和Ⅳ组显著低于Ⅰ组和Ⅱ组(P<0.01);Ⅱ组低于Ⅰ组(P<0.05);Ⅲ组与Ⅳ组比较差异无统计学意义(P>0.05)。③组织学表现,Ⅲ、Ⅳ组与Ⅰ、Ⅱ组比较,尿道全层变薄,结构不明显,肌层萎缩,尿道外膜神经分布(S-100)减少。④盆底肌组织中α肌动蛋白(α-actin)含量Ⅳ组显著低于Ⅰ、Ⅱ、Ⅲ组(P<0.01);Ⅲ组低于Ⅰ、Ⅱ组(P<0.05);Ⅰ组与Ⅱ组比较差异无统计学意义(P>0.05)。结论:结合阴道扩张联合双侧卵巢切除、双侧阴部神经切断能成功建立SUI动物模型,且造模成功率高,稳定性好,较全面的模拟多因素作用下SUI的发生,具有与人类SUI发生机制更接近的特点。
Objective:To establish the animal model of stress urinary incontinence(SUI) through imitating the birth trauma and the decrease of estrogen level combined with cutting off the pudendal nerves,then by identification and compared between models to offer reliable experiment object for research of SUI.Methods:Forty healthy female Wistar rats were randomly divided into four groups,GroupⅠwas the control group,GroupⅡwas the group of colpectasis and ovariectomy,Group Ⅲ was the group of bilateral PNT,Group Ⅳ was GroupⅡ+ GroupⅢ,each group contained 10 rats.the urodynamics examinations of these 40 rats were carried out post-operation on week 1,week 4,week 8.The sneeze test was done to confirm the presence of SUI on week 1,and histological examinations were carried out on week 8.Results:①The positive rats in sneeze test in group Ⅱ,Ⅲ,Ⅳ were 6,9,10,respectively,but none of rats was positive in groupⅠ.②The leakage of urine point pressure in group Ⅲ and Ⅳ were significantly lower than those in groupⅠand Ⅱ(P0.01),and in groupⅡwas lower than that in groupⅠ(P0.05);however,there was no significant difference between group Ⅲ and group Ⅳ(P0.05).③Histological examinations showed that all layers of urethra got thinner,the structure was not obvious and atrophy of muscle layers as well as decreased innervation in group Ⅲ and Ⅳ than those in groupⅠand Ⅱ.④the content of α-actin in the muscle of pelvic floor in group Ⅳ was significantly lower than that in groupⅠ,Ⅱ and Ⅲ(P0.01),and in group Ⅲ was lower than in groupⅠ,Ⅱ(P0.05),there was no significant difference between group Ⅰ and group Ⅱ(P0.05).Conclusion:Integrated colpectasis combined with bilateral oophorectomy and bilateral PNT can successfully set up SUI animal model,and the model shows higher successful rate and satisfactory stability,simulates the occurrence of SUI in multiple factors exhaustively,has a characteristic of closer to the mechanism for human SUI.
