神经外科重症患者肠内营养合并胃潴留的相关因素分析 被引量:60

摘要 目的:探讨神经外科重症患者肠内营养合并胃潴留的相关因素。方法:采用前瞻性队列研究设计,以连续入组方便抽样法把入住神经外科ICU行鼻饲喂养的重症患者分为胃潴留组与非胃潴留组,分析影响胃潴留的相关因素。结果:肠鸣音减弱、低血压或休克史、低血钾、低GCS评分、机械通气治疗、亚低温治疗等为导致神经外科重症患者肠内营养合并胃潴留的相关因素。结论:胃潴留是神经外科重症患者行肠内营养常见的并发症,其发生与多种因素有关。护理中应关注高危人群,有针对性地采取措施消除或减轻危险因素,做好肠内营养护理。 Objective: To explore the related factors of gastric retention among neurosurgery ICU patients with enteral nutrition support. Methods: By prospective and cohort study, one hundred and fifty-one patients were recruited by convenient sampling, observed, and divided into two groups according to the occurrence of gastric retention. Results: The risk factors of gastric retention included decreased bowel sound, low blood pressure or stroke history, hypoklalemia, low GCS level,mechanical ventilation and hypothermia therapy. Conclusion: Gastric retention is the common complication of neurosurgery ICU patients with enteral nutrition. We should pay more attention to high risk patients to reduce the incidence of gastric retention by promoting the nursing skills of enteral nutrition.
