摘要
目的总结经尿道钬激光联合吡柔比星膀胱灌注治疗非肌层浸润性膀胱癌的疗效。方法 2007年8月至2009年8月对52例非肌层浸润性膀胱癌患者采用经尿道钬激光切除术治疗,常规应用吡柔比星膀胱灌注。结果肿瘤完全切除,操作时间15～60min,平均30min,无膀胱穿孔及术后出血等并发症。术后所有患者均获随访,随访时间12～24个月。5例复发(9.6%),再行钬激光治疗。结论经尿道钬激光联合吡柔比星膀胱灌注治疗非肌层浸润性膀胱癌切割精确,出血少,创伤小,术后恢复快,复发率低,是一种安全有效的治疗方法。
Objective To summarize the experience of treating non-muscle invasive bladder cancer,using transurethral holmium laser resection of the bladder followed by intravesical instillation of Pirarubicin.Methods From August 2007 to August 2009,52 patients with non-muscle invasive bladder cancer were treated with the transurethral holmium laser resection of the bladder cancer followed by intra-vesical chemotherapy of Pirarubicin.Results The tumors were completely resected.The operating time was 15 to 60 minutes,30 minutes on average.No serious complications,such as bladder perforation during the procedure and bleeding in the peri-operative period occurred.All cases were followed for 12 to 24 months after operation,and 5 of the patients（9.6%）had recurrent bladder cancer during follow-ups and were retreated with the same method.Conclusion Combination of holmium laser resection of bladder with intravesical chemotherapy of Pirarubicin is an effective and safe therapy,with accurate resection,less bleeding and injuries,earlier recovery and lower recurrence.
